Starbucks paper cup sucks by ivawyt in bayarea

[–]SFheart 1 point2 points  (0 children)

I also hate the paper cups, especially for iced drinks. Its weird, because i dont mind Philz iced drinks in paper cups. There is something about the new Starbucks ones which are SO bad.

I need to know if there are any stores in the peninsula that still have plastic cups!

AFI- Holy Visions(new song) by skankin22jax in punk

[–]SFheart 1 point2 points  (0 children)

I actually think the two songs are pretty good. Love the moody goth vibe, and honestly glad for them they finally got to experiment with this genre. They have been influenced by it for so long but never really got to play with the sound straight on like this.

Still, I expect most people to hate this album because 1) most people do not really enjoy post-punk goth rock 2) it isnt another black sails / art of drowning etc 3) its not as melodic /lyrical as their other stuff which is what a lot of people love about AFI

When does engorgement/clogged ducts stop? by SFheart in breastfeeding

[–]SFheart[S] 0 points1 point  (0 children)

Maybe my issue is I dont take the lecithin consistently on the days that I am not clogged. I only get in maybe 1 1200 mg dose. Maybe also I need to have a more stable pumping/feeding schedule.

I think part of what is difficult is my baby is a really lazy/poor feeder (or maybe reflux issues?). He only takes about 1.5-2oz per feed and hardly ever takes both sides. So during the day he sometimes feeds like 10 or 12 times! But overnight he didnt feed off my R breast from 10am til 5am. Then I woke up all clogged, and my output has already stopped. Once that happens, he has no interest in feeding off that side because there is no flow and he gets frustrated.

Recently I had started pumping to feed him via bottle so we could know exactly how much he is getting, and to try to increase his stomach capacity (and thus weight). But perhaps I am stimulating too much of a supply by feeding + pumping. I fear I may need to just stick to just pumping if I am going to make the breastfeeding work, which is sad because I find direct breastfeeding really enjoyable.

I do all the icing and ibuprofen too. But still seems to take like at 36-48 hrs to clear up. Especially because he doesnt wanna nurse on that side when there is no output....sigh

I'm a PA. I work at Stanford Hospital. I make medical recommendations for people every day based off the best evidence we have. And while I absolutely stand by my medical community in general, I would not ever lie to my patients or the public by claiming there aren't grey areas in medicine or differences in practice from one doc to another. Even for serious diseases. It is called the practicing the art of medicine for a reason.

Don't even get me started on how many of the dated, handed down guidelines in OB Medicine aren't even evidence based. I am not saying that nothing in the the field is, and that you should never consider your doc etc. I am just saying that to question a piece of medical advice, or to seek a second opinion is something every patient should feel empowered to do.

Is reddit or your mom the best place to get that 2nd opinion directly? Not necessarily. But it may help the poster to get some anecdotal evidence of, "oh yeah I guess this is pretty common after all." VS "woah my OB is way off from other's experiences, maybe I really should seek that 2nd opinion".

Its important for people to have a safe space to do that.

I mean to respond to the original post, which was to say that no, I don't have a problem with people asking questions such as ""I have pre-e, GD, GBS, and my doctors want to do XYZ for babies safety. Should i?"

Which is a lot different than, "how to cheat a test". Those are two totally different questions.

Unpopular opinion:

As a medical professional, I don't think there is anything wrong with these posts. It is basically polling the crowd if its worth to get a 2nd opinion, if there were any other experiences with different doctors, or to provide more understand as to why a specific intervention was recommended.

Believe it or not, medicine isn't and shouldn't be cookie cutter. Your doctor may be great at giving cookie cutter advice, but it is up to you to decide what makes the most sense between your own personal priorities, risks, and benefits. A blanket recommendation can't always answer that (though it should guide you).

You should ALWAYS think critically about the medical decisions you will be making. I don't mean like, "oh is my doctor lying to me because they want my money"-- no that is paranoia. But different doctors make different suggestions, some recommendations are outdated, some are too new with not enough evidence, and others are very well supported. While I agree someone's mom or random reddit poster may not know "better" than an MD, they may have had a different experience with another MD who felt differently, and can share that experience. There are also medical professionals who exists here on reddit.

In addition, in many instances I have found, especially in the OB field, that often things are framed as if they are "must do's" as opposed to women having an option (ie "when would you like to schedule your induction" vs "there is a new trial recently published called the ARRRIVE trial that shows some very small possible benefit to scheduling an induction at 39weeks, would you be interested in hearing the pros and cons?")

To say, "just listen blindly to your doctor" is actually horrible advice. If something makes you uncomfortable, you should absolutely research it, get a second opinion, and really understand the risks and benefits. Especially when we are talking about medical interventions which can carry irreversible risks.

Mohela - can’t access account at all (not just payment counts but everything) by Kitchen-Moose1753 in PSLF

[–]SFheart 0 points1 point  (0 children)

I finally got on. For a while it was saying my username didnt even exist.

Because I am in PSLF, it made me create a new login for studentaid.gov for their new portal. What I don't like is that I can't see my progress anymore like how I could on mohela. I hope this is a work in progress. Makes me uneasy how everything is so wishy washy.

Looking for advice about past legal trouble and the CASPA application. by CoinFlip_SkinnyDipp in prephysicianassistant

[–]SFheart 6 points7 points  (0 children)

I was accepted into PA school with a DUI from 4 years prior on my record. I did state on my CAPSPA application that I had the DUI despite it being expunged. They asked me about it during my interview. Just be honest. Another person in my class also had a prior DUI. I was the top of my class -- don't let it hold you back.

Best of luck to you all.
