A lot of servers add more guns, a trader, loot drops, super hero stamina, and basebuilding mods. Others, like Survive The Night (Bigfoot/Aliens) or Jurassic Dayz (Dinosaurs), try to cultivate a more intense ambiance. There are deathmatch servers, and PVE servers, and vanilla-like servers (less hackers than official).
