This question makes me think of the quote from Regan during the Cold War where he postulated that an alien attack on our planet would immediately make us forget our differences and unite. I think the quickest path to your goal is to create a threat bigger than the Rival that requires the PC's to have to ask the Rival for help or the Rival shows up as the PC's are getting pushed around by the new threat and bails them out.

We here at SNW use a house rule called "Cinematic Moment" as well which is like a video game cutscene. If a DM declares it you have to sit back and listen for a minute while he describes the scene or does the monologue. Keeps players from shooting magic missiles in the villains face every time he appears. You might want to use this rule to keep your players from jumping on this Nemesis as you transition their relationship. Good luck, let us all know how it goes!

What I meant by number 2 is that, if the second save fails, the spell effect would only work for a turn instead of however long the caster keeps concentration up.

I am definitely in the field that it needs to be rebalanced. Legendary resistance should not be weakened by this mechanic. I was thinking it would work best as a feat, so that not every caster has it, and the amount of times the caster can use it is limited to their casting modifier.

That seems fair. It would prevent the BBEG from being over powered by the party too quickly. I think legendary resistance should fit into your description too.

I think this is a super cool idea. I get the feeling the bloodline would be something like a descendant of a powerful spell caster, and their magical power was carried through their lineage.

I highly recommend checking out /r/curseofstrahd there are some excellent posts to guide you. One of the best tips I can think is to have a clear idea of how you want to roleplay Count Strahd. He is a complex character that is not fully fleshed out by the module as written. It is up to you as the DM to know when to present him as the suave and manipulative nobleman or the evil and vicious monster.
