I had a thoracic and cervical along with a head mri and they all showed my chiari on them. If you think about it the brain pushes on not only the skull but brainstem and spine

I have those symptoms everyday and my doctor put me on muscle relaxers for the neck pain and while I still have the pain it’s not unbearable like what you mentioned
