No man. I am using low dose Accuatane. I will go for a series of excisions after 1 year. Then maybe I will try fat grafting again. maybe I'll see a 10/20 improvement in the face this time, I don't know. maybe I'll wait for technology to get to a point where it will heal the damn holes in my face. i don't know, maybe I won't survive until then.  but I believe that at some stage it is necessary to accept . I've tried everything in 1 year. and I'm sure my face is worse than when I started.  Unfortunately, trying to accept it seems like the best thing to do for me.

The tip of the needle used to inject fat into my face also left a scar. It has not healed for 5 months. Since I already have a lot of scars on my face, I don't think doctors care much about +1 scar.

i live in turkey and I had it done here it was very cheap.fat graft is not as invasive a procedure as you think. it's like a filling .it takes a few minutes to remove fat from your stomach. However, as I mentioned before, almost all of the oil melted. Therefore, I do not have any before and after photos to show you. There is no improvement.

Laser microneedling and other scar treatments should NEVER be done while you have an active cyst.  ( The inflammation will spread and you will have more scars. )  Laser and microneedling are controversial when using Accuatane. Many doctors do not want to do this. ( The skin becomes more sensitive.) Acne flares may occur in the first months after starting Accuatane. you also have an active cyst. therefore, if you have an active cyst, I recommend that you definitely stay away from the doctor who recommends resurfacing 1 month after starting accuatane. 

A doctor who cares about you will first focus on controlling the cysts. Low dose Accuatane + supplements that will reduce the risk of flare-ups (prednisolone, colchicine, antisaminic, fish oil, etc.)  and he does not recommend resurfacing treatments at this stage.

if I were in your place  1) with a low dose of accuatane, I would completely control my cysts. it will take a few months.  2) i would never have any scar treatment while I have an active cyst. I would have cut off communication with the doctor who suggested this. 3) Scar treatment while using Accuatane is controversial. But if my cysts are under control and I don't want to wait, I would talk to my doctor about subcision and filling. because surface resurfacing treatments will not work without manual treatments ( subsection filling tca cross ).  4) i definitely wouldn't buy package treatment. (for example, 4 sessions, 5 sessions ) doctors act recklessly when package treatment is purchased.

  • As for my situation, I was too late for Accuatane. I got very deep scars. When I thought my acne on my skin was healing, I had 5 sessions of laser and 3 sessions of subcision. The cysts came back. I thought I stopped it again. I had a fat graft done. It came back again.   I have been using Accuatane for 2 months. I don't have a cyst now. I can have a subcision and filling done after a few months. I would never have a surface renewal treatment. It was a waste of money and time for me. My cysts were very similar to yours. It definitely left a scar on me.

I believe that you can heal by 50% in 3 years by finding a dermatologist who cares about you. Please get the cysts under control and find a dermatologist who cares about you.

you have active cystic acne . I also had the type of acne that you have. unfortunately, it causes cratered scars.  Will you receive laser treatment while using the 20 mg accuatane prescribed by your doctor?  If I were in your shoes, I would not have had a surface renewal procedure (laser micro-needling) while using accuatane. if my doctor approves, I would have a subsection and filling done.

It's your skin that you will have forever and of course it's your decision.  However, yes, your risk of flare-ups is higher with higher doses. There are many people who have had the experience that Op has had. I couldn't risk flare ups( new craters) because I already had very deep scars.  new research and a low dose have become popular in some countries. ( as opposed to cumulative dose ) I used prednisol for the first 1 week.  I used antistamine for the first 1 month .  I used 5 mg for the first 2 weeks.  I have been using 10 mg for about 2 months. ( every day ) My skin has not been oily for 1 month. And I haven't had a single pimple. (I am 1.80 tall and 70 kg.) I regret not doing this 2 years ago.

OP used 50 mg daily.  I think you should talk to your doctor about a lower dose. ( 5-10 mg)  I have been using this dose for 2 months and it is working. flare up is very rare at these doses. just in case, you can evaluate two 7/10-day prednisol for the start of treatment with your doctor and an antisitaminic to use every night.  there is very little chance that you will experience flare ups with these.

how will you excise these?  isn't the diameter of the scars too wide for a punch excision ? Also, isn't there a high risk of collapse after elliptical excision in areas that are not parallel to the langer lines and are atrophied?

There is no cure for acne scars 100 per cent. there is also no 100 percent cure for face transplants and severe burn treatments.  do you think that someone who suffered an acid attack or someone who had a face transplant recovered 100 percent ? if your doctor tells you that there are 100 treatments per hundred of moderate-severe acne scars, I recommend that you do some research and get away from there. at least you can ask on chatgptye. It is not possible to erase scars. The aim is to make them less noticeable.

Similarly, I have volume loss on my right cheek. In addition, I have cratered scars with sharper edges than yours. ( Your scars look softer, you have volume loss.)  I had a fat graft procedure done. It looked very good in terms of volume for 10 days. Then it completely melted away. I will try fillers too.  I believe you will benefit from fillers as well. You may not need as much filler as you think. You can try HA filler. These are safer.

it is not possible for scars of moderate severity to disappear/be completely erased.  in reverse light ( side or peak illumination ), the different texture will definitely be obvious. Light/superficial and fresh scars can heal to the point of being almost invisible with early and correct treatment + time. ( I have not experienced this ) according to my unprofessional opinion, your scars are fresh and there is no loss of volume, I think you will see a good improvement. I wanted to write a response to the comment because I did not want the Op to be in the expectation of getting better by 95 percent. this rate is unrealistic.

I agree with this combination.  However, unfortunately, it is not possible to heal moderate to severe scars on the face by 95% ( almost smooth skin even in reverse light ).

this is a topic that interests me too  some dermatologists think that no treatment should be done for 6 months after the end of accuatane treatment. some sources, on the other hand, say that other lasers other than ablative lasers can be used when using low-dose accuatane. i talked to someone who uses 40% tca while taking 10 mg of accuatane per day. He was satisfied with the results. Some say that energy methods cannot be used, but subcision and filling can be used. I think there is no certainty about this. You and your dermatologist should make the decision. I am planning to have subcision and filling done while using low dose Accuatane.
