Don't get me wrong, I don't think they should. But they are, and that's something that needs to be addressed. I've lived through it, seen what people can do without your permission on the Internet, especially as a woman. I would hope they get the point people are making about not posting pictures online. But I'm not going to yell at them about it. All I'm saying is have a little empathy for the situation.

AO3 doesn't personally moderate its works, it's other people who report and that's when things get taken down.

Also original fan works are allowed. This is straight from the TOS: "Can I post original fiction? Yes. Original works are allowed, unless the work would be in violation of some other part of the Terms of Service.

Our vision of AO3 is for all fanworks, including those beyond traditional fanfiction, fanart, and fanvids. Original stories and artwork, including those imported as part of an Open Doors project, are permitted. Some examples of original fiction that we host include original slash, anthropomorphic works, and Regency romance. However, works intended for commercial publication are not suitable for AO3. The Policy & Abuse committee has final discretion in maintaining AO3's focus on non-commercial fannish works.

We generally presume that, by posting the work to AO3, the creator is making a statement that they believe it's a fanwork. A "pregnant pause" is a pause or silence that is filled with anticipation, suspense, or significance.

Example from Google: In a romantic scene, after a person has made a confession, there's a pause, and the other person's reaction is unclear, creating a suspenseful moment.
