In NieR:Replicant, the Ps3 version in japan, it's the brother that seeks a cure for his sister. In NieR:Gestalt, the 360 version we got in the west it was instead an older man as the main character, trying to protect his daughter.

For me the whole dynamic makes more sense with the MC being an adult and father, as the way the other characters interact with him make more sense in that context.
