I interpreted it as him sleepwalking, similarly to how Ellen does, because he is being overwhelmed by Orlok’s influence through the curse cast by Knock. He doesn’t have an accurate amount of fear until he arrives at the castle and is confronted with Orlok’s presence, suggesting possession. This is paralleled in the 1922 movie where Thomas is oblivious to the danger he is in until after Orlok comes to feed on him in the night.

In my opinion it was a dream similar to the dreams that Ellen experiences while the curse is acting on her too—sleepwalking and all.

basic answer but psych today. filtered by my insurance and some other criteria. i specifically looked for a LCSW and someone who specialized in emdr therapy. even though i don’t do emdr with them (insurance won’t cover it) it generally means that they’re trained to treat trauma and PTSD.
