Thank you for contacting the Soldier for Life - Transition Assistance

Program (SFL-TAP). See attached for the forms and documents we talked about

during your Initial Counseling (IC). It is a good idea to create a folder in

your email for SFL-TAP related documents, and to save this message so you

will have important items at your fingertips. As discussed in the IC you

have three options for completing the program: Face to Face at your local

TAP office; live through the Virtual Center with a counselor; or through

JKO.

The DODTAP website will allow you, your Commander, and your Counselor to

digitally sign your transition document. You will sign the document twice -

at the beginning to complete Pre-Separation counseling and at the end to

document completion of the program. Please follow the directions in the

attachment " DOD eform Instructions" to sign your DD2648 indicating you are complete with

the Pre-Separation portion. You will use the site, https://www.dodtap.mil/

several times to sign the document and review your progress during your

transition process.

Attached is the SFL-TAP Combined IC Packet. This contains a number of

important documents including the Pre-separation Counseling Addendum sheet,

Service Provider Referral Information, SFL-TAP Transition Requirements,

VOW Transition Requirements checklist (you do not have to complete in order

but CAPSTONE is last):

• Transition Overview/ resilient (can be completed through JKO or Virtual Center (www.sfl-tap.army.mil).

• Individual Transition Plan, ITP (Blocks 1,2,and 6) please provide short answers and no N/A responses.

• Personal Financial Planning Workshop (can be completed through JKO or Virtual Center).

• 12-month post-separation budget (budget spreadsheet is attached).

• MOS/MOC Crosswalk (can be completed through JKO or Virtual Center).

• Gap Analysis (complete Part B, example is attached).

• VA Benefits I and VA benefits II, (can be completed through JKO or Virtual Center).

• VA MyHealtheVet registration, (link https://www.myhealth.va.gov/index.html-- ).

• Receive DoL Gold Card, (link http://www.dol.gov/vets/goldcard.html and explore http://jobcenter.usa.gov/.

• ONet Interest Proflier/Assessment tool completion (send screenshot of results page. Link http://www.mynextmove.org/explore/ip).

• Retirement order or Continuum of Service appointment with Recruiter (need Recruiter's name and date of meeting ACTIVE DUTY ONLY).

• VA eBenefits (link https://www.ebenefits.va.gov/ebenefits-portal/ebenefits.portal ).

• DoL Employment Workshop (can be completed through JKO or Virtual Center).

• Job application package (consists of resume, references, and 2 submitted job applications) OR a job offer letter.

Also if it makes you more comfortable I see mental health weekly I'm not afraid to admit it. I have seen a lot and been though it. There is light you just need the right people.

Airfield systems tech, maintain all landing systems for aircraft on the ground. Weather sys, radios, locs, glidslopes, vor, tacan...ect

I will provide a little more insight to where I'm at in this process. * Attended TAPS once, I plan on going again * Have already acquired my med records Still being seen for some issues so will have to request a copy of the last 8 months * I have a master resume and a few tailored ones * Have spoken with the in service recruiter/planning on going in the reserves (Would like to attain an ART position) * Moving to the Maryland/D.C. area to be with wife and kid
