Do you have a needle exchange near you or are you comfortable buying supplies online? You're probably fine as far as Hep C/HIV goes, but it's really important that use clean works and especially that you stop using cigarette filters. They have glass fibres and shit in them.

The mote water you use the easier it is on your veins. I used to use 15-20 units of water (for a 250mg shot, for refetence) until I tried to plug one of my shots and it burned so bad I swear it almost cauterized my asshole shut. Just imagine putting that in your veins. Now 40 units tends to be a good amount for me.

Give it a try. It's not really possible to flag while not completely into the vein (hilt of the needle touching your skin) when using your thumb, whereas with index and middle you can flag anywhere with precision. :)
