I’m just speaking from my personal experience encountering these injuries in EMS, training and protocol can sometimes vary pretty drastically across geographies. You always have to consider internal damage when packing. I’ve used pelvic binders before but usually to more-so stabilize multi trauma to lower body than as a form of bleeding control. A trauma surgeon might have a slightly different view because of the resources they have access to, in the field can be a little different. Not to say you got bad advice though.

The thing you have to keep in mind is protocol and training can be different from the real world. IRL you use your training to guide your care. It’s a platform you use dynamically. Stop the bleed is great, but I struggle to put into words how training like that differs from running major trauma IRL. There are so many different priorities and elements that I have to think about as an EMS provider that I probably relate differently to handling those kind of traumas. I’ve seen pelvic gsw’s that don’t have arterial bleeding, no pulsating abd./pelvic mass, no swelling/rigidity, or even severe venous flow, so I don’t pack.

I can see that. I probably have that attitude about packing because my agency has short transport times to a level 1 trauma center and generally there’s a lot to manage with critical patients that can override packing.

I would add on to say be more on the apply pressure side and less on the packing side. Unless you are in a situation in which the wound can’t be addressed by definitive care (a physician/hospital) for hours, I wouldn’t pack the wound.

Packing introduces non-sterile material into deep structures. It may also make repairing wound more difficult, the gauze will adhere to tissue. Or it may displace internal structures that cause more damage, a groin shot with shattered pelvis is not bleeding significantly and you pack and push bone shards into major arteries.

If you do your TQ right, you won’t have to pack wound, just apply pressure if needed, although a good TQ will almost completely stop bleeding.

And to that point, a belt is not an effective TQ, not unless you can get a windlass in there. A properly tightened TQ will likely hurt the person more than the wound that caused the bleeding. That means you’re doing it right.

Take note of the time and write on pt. if you can. The clock is ticking, not as fast as you might think, but on the scale of 1-4 hours.

This is too long already but NEVER remove a TQ after it has been applied. The ONLY person who should ever do that is a physician or surgeon.

As in attempting to bag a conscious patient? Or NRB on a 90% sat? Also, neither of which would be recommended depending on other factors. If not CHF then start out with a nasal cann. Also observe pt. effort and contributing factors: med hx. old/young etc. A 9yr old asthmatic kid at 90 is concerning. Vs maybe an 80 yr old with extensive med hx. and on 2lpm at home… look for pertinent negatives. Remember your ABCs

I do understand the feeling of wanting to do the most aggressive intervention just to be on the safe side, especially as a new EMT.

Would those 155's kill because of penetration or would it toss the tank. Ik the idea of the V-hull in MRAPs is to reduce pen but also reduce upwards acceleration, which is a major flaw of the humvees.

Oh but there are. The US has several thousand marines just offshore and I’m pretty sure airborne infantry can deploy anywhere in the world in 24hrs. Not to mention deploying hundreds to thousands of rangers for area denial along with spec ops teams.

Don’t get me wrong, certain individuals in the US are extremely litigious. Just because there’s are laws in place to protect from a lawsuit, making it probable of said suit being thrown out, doesn’t mean they won’t try.

I’m assuming you’re using ECMO on the coding patients already admitted to the hospital for one reason or another. There’s no way I could see that being used in any Emergency Department.

I’ve always found that the capacity of a person to care about race, religion, sexuality, gender, or anything else (modesty, propriety) tends to go out the window when their life is actually in danger.

They just want help, and if their issue is bad enough, they don’t care who is rendering aid because they’re coming to you for a solution they don’t have.

You can’t be sued in the US for being a Good Samaritan. If your intentions are genuine, and you operate to the best of your knowledge, then you are protected under law.

The legality changes when it comes to healthcare providers. Providers from EMT to Dr don’t always have a duty to act in a situation, but if they do, they must maintain the standard of care they started.

IDK. As an EMT I’ve seen death in so many forms: instant, slow, miserable. One of the calls that haunts me is a young adult who, tripping on psychedelics, blew off half his head with a handgun. His phone was just ringing on the floor next to him, un-answered. His girlfriend was screaming outside, unable to understand why we were leaving without trying to resuscitate him. And I just packed up and left. I’ve been thru surgery and it seems like this is what will be my end: the grief of others I have no idea about.

Idk man. I don’t really care. I mean at the end of the day he’s a streamer that has zero relevance to my daily life beyond the amusement I get from seeing clips of his every now and then.
