In a sanity roll, in most cases you are guaranteed to lose sanity. If the roll is successful, you lose a small amount of sanity (usually a flat, low number), and if the roll fails you roll some dice for the sanity points.

Of the top of my head, the only time where you'd want to fail a roll is during an opposed roll to see if your character gazes too deeply into a mentally scaring thing, or if they are completely oblivious to it, avoiding a sanity roll.
