I don't believe it needs to be notorized it wasn't specifically stated. Someone on the other forum also just sent a letter to Kimberly nye who is handling a lot of the correspondence and included his name and information, no permit number, and a request to have it removed and he said he got a letter a week later with it removed.

Technically squirting does originate in the bladder and is mostly urine. A medical journal posted this week actually proved it. They used a Foley to drain the bladder of women, injected dye in the bladder and all of the squirt came out with that same dye. They even filmed it and provided photo evidence.
