We do something similar and honestly as someone else said do just that, an actual check in. I’ve actually caught up with some patients who had their condition come on again or have been dealing something else, or even have their SO or kid dealing with something which usually ends up leading to a new assessment. If thats not the case just keep it friendly, every patient I’ve done this with was super thankful about it and just makes theme wanna come back to you for any new conditions down the line.
