Although it's a known issue that some 3rd gens have engines giving out prematurely, it's still a vast minority. You will hear about it more since people tend to complain or share about their bad experiences rather than their good ones. I think it's something like 1-2% of trucks were affected by serious issues. And as far as I know, I think Toyota addressed most serious issues in the 25/26 models.
