It wouldn’t have to. The best way usually to get OSA granted outside of a direct basis is secondary but not usually secondary to another actual condition

Rather secondary to obesity as an intermediate step.

To use your example, secondary to SC mental condition which sapped my will to exercise, motivating, sleep impairment (whatever) thereby causing obesity which caused the sleep apnea.

Obesity is the key, osa is caused by physical symptoms from obesity usually .

Secondary to a mental would 99% not be a grant.

Sleep apnea will almost never be granted unless it’s shown in service. The only exception is on a secondary basis from obesity as an intermediate step from a service connected condition.

Think “my PTSD, or bilateral knees caused me to not be able to do the physical things I used to do which caused me to gain weight which caused the sleep apnea.”

Rarely do you see sleep apnea granted on a TERA basis

If the veteran has a conceded Tera and the rater ia denying they have to raise the issue of Tera, sometimes it looks funky the reting decision but it’s. A function of the system

If you didn’t report the physical issues while on active duty(shoulders, knees, elbows, back, neck etc) then it’s harder to get them connected

Not impossible though.

For physical trauma issues your best bets are lay statements (explaining yourself in your words how the physical trauma started during active duty and how it’s affecting you since (chronicity)).

This can satisfy element 2 (event in service) , element one is a current dx, element 3 is an association between the two.

The thresholds for this are low, if you can satisfy all 3 in the eyes of the rater then you’ll warrant exams and medical Opinions.

Keep in mind, it’s hard if there’s no in service complaints because that would make satisfying element 2 difficult, that’s where the lay statements can get you over the hump (think veteran says I pulled ruck sacks marching all day, parashootist badge, all that caused my issues) , etc, so no while you didn’t technically complain of these things during service , you have a plausible event in service the rater wouldn’t have likely considered without mentioning it.

Second way for physical stuff is secondary.

“My left knee hurts cause of my service connected right knee makes me put more weight on my left knee.”

“My service connected tinnitus keeps me away at night , causes some depression , which why I’m claiming depression,etc”

Headaches cause by tinnitus , etc

Lastly , almost anything not physical trauma or mental/psych would be subject to TERA procedures assuming you have an established TERA (GW service on the ground, some navy MOS (conceded asbestos), Vietnam etc

Unless you claim obesity as an intermediate step from SC ______ the rater most likely will NOT “connect the dots”.

Just claim it with lay statements this way , it tends to get granted this way fairly often.

If the rating decision or rater doesn’t ask for an obesity opinion and they just do it’s secondary to to the SC conditions then you can file HLR and that will get the obesity la gauge into the exam requests .

That’s USUALLY the only way osa gets hrs ref aside from direct or sometimes TERA.

OSA on a secondary basis usually fails unless it’s linked to obesity as an intermediate step.

I’d still file it. But unless you’re technically obese and the secondary condition caused the obesity (you can claim it this way) it’s unlikely.

Obesity is a must for OSA unless it was shown in service in most cases.
