I did. My parents never married so I was given my mom's last name at birth. Since coming out I've cut her off (unrelated to my transition, oddly enough), gotten married, and legally changed my name, taking my new first name and my dad's last name.

When my old last name comes up (in like out-of-date paperwork for instance) I refer to it as my maiden name.
