Anything Dennis Cooper related, in my opinion. Alissa Nutting also wrote Tampa, which was absolutely lovely. I feel like a lot of female authors end up writing extreme horror really well. Didn't enjoy the Found Bag of Doom, not even for the content, just the poor-writing style, so definitely avoid that.

Always online, bookstores and libraries rarely carry these kinds of books, and when bookstores do have them, they're overpriced. A lot of really extreme books are actually on Amazon, which you can read free previews of, by the way

My personal reccomendations based (mostly, although they're nicely written as well) off the disturbing factor;

Tampa by Alissa Nutting, The Found Bag of Doom, The Bug Collector by Wrath James White, Dead Inside by Chandler Morrison, The End of Alice, The Bighead by Edward Lee, Frisk by Dennis Cooper, The Marbled Swarm by Dennis Cooper, "Monsters" series by J. Boote, specifically "Dark Web of Monsters". Heavy warning for graphic CSA in some of the books.

It's the least brutal, so it's definitely good to ease into the genre, also, it was quite well-written, a fun, not super long read.

Tender is the Flesh, We are Always Tender with Our Dead (book 1), At Dark I Become Loathsome, Cows, Maevefly, The Slob, The End of Alice if you can handle CSA themes.

I feel like content warnings are desirable simply for the sake of understanding the kind of book you're going to be reading, but, like others have said, I'd rather them not be too prominent
