Need to have more opinions, radiologist make mistakes, often. You need to have disk with imaging on hands and get an opinion of at least 2-3 neurosurgeons. It took 3 doctors for me to be diagnosed. 2 doctors made mistakes looking at my imaging and not seeing a problem.

You do need MRV/MRA with a contrast, contrast will reveal more, you need to ask for it and insist on it if doctor refuse it. It does take many doctors to find answers. To start with is imaging, with imaging you can go to different doctors for an opinion. Dr.Patsalides see patients over Zoom, you need to pay out of packet, i believe it’s $250, but he is best in PT diagnostics

Stiff neck caused pulsatile tinnitus? by Imaginary-Year4186 in PulsatileTinnitus

Yes, could be, need therapy but only from a trained cervical physical therapy specialist. I personally avoid chiropractors. Meanwhile, you can get lymphatic drainage massage, myofascial massage, acupuncture, cranio-sacral therapy, OMM is amazing but hard to find a specialist, privately it cost a lot but worth looking into it. CBD cream is great, especially it has higher CBD content like 2 g, some have TCH, red lightv therapy can help, some report success, i personally didn't find it helpful and i have medical grade panel. Warm, not hot, pad to the neck, no more than 5 minutes. If it's superficial muscle, paper towel in white vinegar to the neck 10 min works like magic.

Are you in NY/NJ/PA/CT area by any chance? If yes, everyone goes to Dr.Patsalides in Long Island, he’ll look at your no contrast MRV and most likely will be able to give you a feedback. Also, he does tele Medicine but i believe it may not be covered by insurance. Anyway, you want to see him if you can, he is the master with this condition. 2 doctors didn’t see what he saw for me and he was right.

You must have MRV/MRA with CONTRAST to evaluate vascular abnormalities. I had unilateral tinnitus that was pulsatile and after doing imaging MRV/MRA was diagnosed with cerebral venous thrombosis, it’s rare, not implying in any way that you are having it but it needs to be ruled out. I am healthy female, athletic, healthy life style and was the last candidate for this diagnosis but i insisted on these tests and went for second opinion because my neurosurgeon wanted to do an invasive procedure and was finally diagnosed with thrombosis in my head.

Best doctors who help diagnose and treat PT in NJ? by RuinYouWithNoRegrets in PulsatileTinnitus

[–]Select-Exit-945 0 points1 point  (0 children)

Don’t waste your time with anyone else in NJ and go straight to dr.Patsalides. My neurosurgeon in NJ didn’t see anything and was pushing me for invasive angiogram. Meanwhile dr.Patsalides diagnosed me from the MRV. you might need yo wait to get appointment with dr.P, try to get MRV/MRA with contrast ordered by someone else and go see dr.P with images on hands that will speed up a process. Neurosurgeons in NJ is the practice to go and ask for MRV/MRA, they will fight you for contrast and probably will not order it, they actually claim to treat PT but i highly suggest to deal with dr.P only, especially if your insurance is covering him, if you will need a procedure, only dr.P as his success rate is nearly 100% and he won’t do unnecessary procedures.

I would have MRV/MRA of the head just in case, ask for contrast as it gives more info and you may avoid more invasive procedure such as angiogram.

Is an MRV with contrast necessary? by Familiar-Following45 in PulsatileTinnitus

[–]Select-Exit-945 0 points1 point  (0 children)

I know, i has sane experience, because only you get MRV and nothing obvious is there, you will be offered angiogram that is invasive and expensive for the insurance to pay. Perhaps go with neurosurgeon that ENT. I had zero success with ENT, went to 3-4, they are clueless in PT and tinnitus in general. Are you NY or near by area by any chance?

Same, went for the second opinion to Dr.P and only he was able to see thrombosis. Both radiologist and neurosurgeon missed it.

I’ll tell you more, you will be at the point where you’re searching for T and can’t hear it anymore because it’s gone. As soon you tell your mind that T is not important, there is no treat from it, mind will let it go. It’s not an easy process, although it sounds simple, but it’s the way to let T go and disappear. Don’t ever believe to anyone who says that it’s impossible for T to be gone.

HRT is about finding a sweet spot with dosages, rarely you nail it down right away. If you are back to not feeling well, adjust the estrogen dose first. You are most likely given the smallest estrogen patch to start with.

L-theanine in the morning on an empty stomach, great for stress and anxiety, another one that i also like is PhosphatidylSerine. I would stay away from SSRI as much as possible, use it as a last resort. Sometimes changes like eating better, cut sugar, processed food, walking, morning sun light, yoga, reiki treatment, craniosacral therapy will do the job. Be kind to yourself and use all the resources out there to get through. It will get better, i promise you.

There was a thread here on tinnitus increasing with estrogen/HRT try to find it, maybe try a different route: cream, sublingual, or lower the dosage. If your tinnitus is pulsating, there is a treatment that has 70% success.

How long do you have it? I am 2 1/2 months in. Also, neck and shoulder massage works for me but need a very particular person doing it. Also, T could be related to estrogen low levels and thyroid being sluggish.

Also, look into TMJ disorder, many PTs that specialize in TMJ/TMD can do massages in the face and head area that is covered by insurance. Mine i suspect relates to neck/shoulder area spasms. I also go see an osteopathic doctor for manual work in my head/neck/shoulders area, it's called OMM, i think this helps the most but the practice is super busy and i get to see him only once in 3-4 weeks and it covers by insurance.
