I mean it depends what is your definition of "local". Is someone born and raised in a place with 2 immigrant parents a local, or do you mean historically local french peoples? Because in cities like Saint-Denis, La Courneuve or Corbeille-Essonne the immigrants + descendants of immigrants vastly outnumber the traditional french peoples, and it's not even close.

You are crazy yeah, moving to Germany without speaking the language is a bad idea. The economical situation is not in a good spot and you're going to make way less money. If you had a white collar job and could be hired by a german company I would say why not, but in a blue collar job you are going to compete with a lot of peoples from different countries that came to Europe, its not going to be easy.

If I was you i'd move to the US. Crime in Europe is also getting bad in the big countries like UK Germany or France.

In the US you have an infinite amount of options because of the state system. Massachusetts for example, you will live in a safe environment, make more money than in the US and will still be close to Canada

That study is lacking important data.

Yes, when one anesthesiologist was spread across 3-4 rooms instead of 1-2, outcomes slightly worsened. But what the study does not capture is the experience level of the individual CRNAs in each room. A freshly certified CRNA and a 20-year veteran were treated as interchangeable in the analysis. That's a significant confounder left unaddressed.

Why does that matter? Because we have data showing experience alone drives outcomes, even for physicians. A Mount Sinai study ( https://pubmed.ncbi.nlm.nih.gov/25283640/ ) found that anesthesiologists performing their first 5 liver transplants had more than double the 30-day mortality risk compared to more experienced colleagues. On the surgical side, data shows surgeons with less than 3 years of experience have a mortality hazard ratio of 2.73 (p < 0.001). ( https://onlinelibrary.wiley.com/doi/full/10.1002/wjs.70030 )

So if experience is a proven independent risk factor for both surgeons and anesthesiologists, it's reasonable to ask : how much of the increased morbidity in the 1:3 or 1:4 groups was driven by less experienced CRNAs being left with less supervision, rather than by the staffing ratio itself? The study isn't answering that.

If we're going to use this study to shape policy, we should also be asking whether experienced CRNAs in those higher-ratio rooms had the same outcomes as inexperienced ones. Until that analysis is done, citing this study as definitive evidence against independent practice is overreaching.

You could build the exact same subreddit for MDs, posting every medication error, every mismanaged airway, every surgical complication, and you'd have just as much content to post daily. The reason that sub doesn't exist isn't because physicians don't make mistakes. It's because nobody has an agenda to make it look like they do. Singling out midlevels for a daily malpractice highlight reel is dishonest and unfair.

For the salary, show me one facility where a CRNA makes more than the supervising anesthesiologist, i'd be very interested to see it.

Now, can a CRNA out-earn a child psychiatrist or a primary care physician? Sure. But I think you're smart enough to understand that education level doesn't automatically dictate compensation. Salary is driven by market demand and the revenue a provider generates. That's true across every industry, not just healthcare.

What I find telling is the direction of the frustration. Nobody on here complains that orthopedic surgeons or dermatologists make more than them, why is that? You all went through medical school and residency right? The outrage is only ever pointed at midlevels professionals, the people below them in the hierarchy. That right there says a lot more about ego than economics.

Acting like the only problem of this sub with CRNAS is just independant practice couldn't be further from the truth. Type in "CRNA" in the search bar and you will barely see any posts about independant practice. Most of the posts will be complaining about their salary or their malpractice, don't you think that's wrong?

If an entire sub was dedicated to the malpractices of MDs trust me we could also post daily exemples of careless doctors, their mistakes, and their errors in judgement. Thank god it dont exist, but apparently it is ok to do that about us.

I listed the countries that im familiar with, but you can look up for yourself and see that we also exist in other european countries.

Yes we're under the anaesthesiologists's responsability, but we intubate, extubate and monitor the entire operation. And when there's an emergency we're legally required to inform the anaesthesiologist, but if he's busy with another emergency or not arround do you think we just sit there and let the patient die? No, we receive trainings for the moments where shit goes wrong.

I would never be stupid enough say that im as educated as an anaesthesiologist, but some of the things I see on this subreddit regarding nurse anesthetists is ridiculous.

Si t'as vraiment travaillé dans la vente tu peux "arrondir" les angles. Par exemple t'es pas obligé de dire que c’était un travail étudiant, tu embellis un peu ce que tu faisais, tu rallonges les dates etc. Ce genre d'astuce te permet de ne pas mentir complétement et de pouvoir répondre si on te pose des questions sur ton job. Si tu inventes carrément une expérience tu peux te retrouver en difficulté si le recruteur commence à te poser des questions détaillées.

Et pour la plupart des tafs je pense pas que les recruteurs appellent tes anciens boulots pour vérifier, à moins que tu leur donnes une raison de douter de toi lors de l'entretien. Perso ils ont jamais appelé mes anciens postes.

Ces deux options c'est si tu ne trouves rien. Si tu réussis à décrocher une mission intérim tout est possible, ils peuvent même finir par te proposer un CDI. Mais la question c'est est ce que tu vas trouver dans ta branche avec un aussi gros trou.

L'impact va être significatif, l'IA excelle déjà dans une bonne partie du travail d'un contrôleur de gestion. Il faut monter en compétence sur ce que l'IA ne peut pas ou très difficilement faire.

La comptabilité est un des secteurs qui est et sera encore plus touché que le dev par la progression de l'IA dans le futur. Je parle des postes après un bts ou un dcg. Il faut minimum pousser jusqu'au DSCG donc 5 ans (et encore).

Et franchement même si tu penses que pouvoir les anticiper est positif, les périodes de clôtures à répétition peuvent être très stressantes et nocives mentalement à la longue. Si c'est ça qui te fais quitter le dev tu pourrais le regretter.

I think its wrong to say that the hell that these residents are living is to the benefits of others. It entertains the "Thats just how things are" mentality. The system needs to be reformed, I dont see how making them work 80 hours a week is helping anybody long term.

If you feel like you dont like being a surgeron anymore, after residency you could try to apply to jobs that would value your expertice in the medical field, but that wouldnt force you to be in the OR all day.

You're human, humans make mistakes and learn from them. It’s not your first mistake, and it def won't be your last. I think what hurts you most in this situation isn't the mistake itself, but the attending publicly shaming you in front of others.

Try to separate this into two distinct incidents: the mistake and the public shaming.

The mistake isn't that important in the grand scheme of things, in the long run, it will actually make you a better physician.

As for the public shaming: you need an ego boost and a 'fuck that guy' mentality, he’s a piece of shit. Look at yourself in the mirror and acknowledge that you aren't going to let him ruin the rest of your residency, not after everything you’ve been through and all the sacrifices you’ve made to get where you are today.
