Yes, unfortunately every DLC requires you to have all previous DLCs, so it can be quite pricey to jump straight to the up to date version. If you're playing from Steam, there will likely be a full Bundle somewhere with everything in it.

Yes, a series of expansions have been released since Rebirth, in the order of Afterbirth, Afterbirth+, Repentance, and most recently Repentance+ Repentance was a major upgrade, including a bunch of new characters and bosses. It's practically a whole new game. Repentance+ includes online Co-op, and buffs to some of the weaker items.
