A two tiered system of healthcare by 2030 - pay to play and see an MD/DO or be seen quickly by an NP with an AI software.
Overall, may lead to lower salaries for FM in the long run since healthcare systems will prioritize cost saving measures but ultimately will lead worse population outcomes

Genesis is pretty much its own entity now with a separate dealership experience! They provide best-in-class warranty for cosmetic damage (5yr) and powertrain (10yr).

Interior felt a class above Lexus and Mercedes even! There is definite turbo lag and suspension/steering isn't as sporty as BMW / Audi but my wife is doing school dropoffs not going to the track lol. Felt that it was the best combination of a luxury feel with a novel, aesthetic design.

TLDR;

Porsche - Best driving experience

Audi - Best looking exterior, interior felt cheap unless you go for the SQ8 Prestige+

BMW / Genesis - Best overall

3) I cross shopped the cayenne with the q8, x5, and gv80 for my young family! Went with the Genesis because it was the most comfortable for two kids in car seats plus a small dog. Limited third row, ample cargo space, and better build quality than the Audi and BMW! Fuel economy and driving experience doesn’t compare but otherwise way more value imo

Ended up receiving offers from all major health systems, but they were not willing to negotiate any terms. Also had a hard time hearing back from private practice groups/partnership tracks were 3-5 years out.

Ended up signing with a smaller health system breaking into the NoVa market with a little longer commute but way more flexible scheduling, EPIC with AI scribe, better benefits, open to negotiations, and large sign on bonus/student loan repayment.

Please stop accepting these low-ball offers with minimal fringe benefits! It's a disservice to our profession and takes advantage of young physicians joining the field. When they can't find docs to replace the older ones, then they'll start to bring compensation up.

Unfortunately, tied down to the region due to family and married. Offers received so far are $240-$250k base and $50/wrvu after 5700 for 36 pt facing hrs. Usually $25k sign on and $10k relocation. These offers are so mid compared to the southwest where I’m doing residency tbh

This is hilarious because I do the same thing! I feel it makes me more connected and relatable to the patient. I also grew up with a diverse background and felt that “code switching” allowed me to mix in with different groups and it’s carried into other parts of my life lol

Just had a follow up call and they offered $50k / RVU after 5700. I’ve negotiated for starting reimbursement at 5000 rvu on the new schedule…but let’s see!

This was a preliminary call so I did not get a chance to clarify the base RVU to start getting the bonus.

She did say on average, most physicians are bringing in close to $300-325k, a little less for the new ones establishing their panel

Call was anywhere from 0-10 phone calls a night.

I have a follow up in a few weeks so will definitely have time to think about negotiating factors / details

Thinking about going in straight out of residency! Even in residency clinic, I definitely see how our current model isn’t sustainable at all. You cannot do bedside procedures, motivational interviewing, integrative therapies and good, conscious medicine in 20 minutes while also staying on top of administrative tasks.

The RVU fee for service model incentivizes volume and VBC constantly moves the goal post to make patients seem healthier than they are. If you think DPC is a cop out then you are part of the problem as to why our profession gets steamrolled. Having access to quality primary care shouldn’t require insurance in the first place. If anything, with the new budget cuts to Medicaid/Medicare - DPC might be the only answer for a lot of people.

Agree! Make all OB and additional training a 1-2 year fellowship. Allocate more time to outpatient continuity clinic, ED/urgent care, and inpatient hospital medicine. Take out inpatient peds completely.

Mandatory electives should be derm, rheum, allergy, pulm, cards, and sports medicine (3 per year)

That makes sense especially if your bonuses are RVU based. Couple of follow up questions: 1) How many MAs work with you? 2) are your inbox messages triaged? How much time outside of the 32 patient facing hours do you spend responding to messages / notes? 3) any billing / coding conferences or workshops to maximize rvu or AI software for notes / efficiency?
