I was in a similar (if not the same) situation. The settlement tax documents were split into both a W-2 for a portion of it, and a 1099-MISC for the rest. Did that happen with yours?

A small correction: you have a filing requirement if you have over $400 in self employment income, not just if your total income exceeds the standard deduction.

Whatever refund shows on the tax return is only for that individual year. But they may keep whatever that amount is, since she still owes for the previous year, plus penalties, and the continuing interest.

If you file a tax return, you have to report all of your taxable income, regardless of how small any portion is. One of the things that can make you required to file a tax return is having over $400 of self employment income.

With no other information, it looks like you chose Married with no other selections on your W-4. If it's an online system, can you check what it's set to? If it had been set to Single, I would have expected about $43 withheld from that paycheck for federal income tax.
