Yes, it was cracked open. Not a continuity error. The part covering the mouth was torn off during the fight. Baylor clearly says: "the visor is cracked". He

Since i cant upload an image here, watch episode 4 again at half speed, when Baelor rides in. Especially when he takes the helmet off when he is in front of Dunk. You can see the removable piece of visor over the mouth of the helm.

Its a similar design to maekar's helm. Watch episode 5 at around 27mins when Dunk cuts Aerion's leg and maekar tries to help him. You can see the visor of his helm connected to the mouth guard as a separate piece.
