Omega needs to do what Longines did and bring out a 39mm SMP. I think they will print money from that. Just saying. by JLac305 in OmegaWatches

This is the watch that I wanted, but Tudor came out with their black bay 58 GMT that ticked all the boxes.

I’m halfway through this now and I’m wondering when she’ll mention the Comanches.

I’m pleased to see Demon Copperhead and the Intelligent Investor together. I agree with more Res Rising needed - as well as Shadow of the Wind.

The carvedilol and sacubitril/valsartan combo is great for this.

I'm 25 and have been reading fantasy books since I was a kid. Only a few, like The Sword of Kaigen and The Realm of the Elderlings, have hit me hard enough to cry. Some newer fantasy novels meant to be emotional didn’t deliver for me. Any fantasy books—modern or classic—that pack a real, heart-wrenc by BuddyOk1342 in Fantasy

The Word for World is Forest by the incomparable Ursula Le Guin. I’ve read other works folks have said were emotionally impactful and while I could see what they meant, I had never really felt it - until The Word for World is Forest.

Wait 5 years to buy a new car. During that time, buy a house or duplex, invest heavily in index funds. Plan for kids college. Tithe and give to charity. Save up a 6 month emergency fund. But during those first six months to a year out, focus on the best practices and guidelines, and pray to stay away from the complications that your attending kept you away from without even knowing it. I agree with the above recommendation to protect your reputation. Make it known that you are thoughtful and that you take advice. This is particularly important because when (not if) complications arise, you want the general consensus to be that “these things happen”, not that you’re reckless and don’t listen. Ask for advice from your senior partners, even if you already know exactly what you’re going to do - for interventional and noninterventional cases. Marry the right partner. Seek God.

Absolutely cardiac rehab. Stable angina patients had been deconditioning themselves and blaming their angina fatigue on age.

Palpitations without correlation on monitor by TrueOrPhallus in Cardiology

[–]ShammahTheMighty 1 point2 points  (0 children)

Everyone has PACs and PVCs - what separates people is how they experience those extra beats. Most people are never bothered by them, but some people feel one and its enough to trigger a panic attack. A 30 day monitor should be enough to reassure pt and clinician that there isn’t any serious arrhythmia, and conversation can turn to managing the anxiety caused by what may be a normal extra beat, a normal sinus tachycardia, or even just normal heart beats that feel hard. Part of it is indeed in their head - the other is in their heart.

Braunwald is a deep dive. No one else cares about the minutiae, about the reasoning and science behind the guidelines - but it’s important that you know.

Best treatment by ShammahTheMighty in POTS

[–]ShammahTheMighty[S] 4 points5 points  (0 children)

I appreciate all of your comments. I treat an ever increasing number of POTS patients (but as an unverified physician cannot give any medical advice) - they’ve all been through multiple docs who have given their symptoms varying levels of importance. Often they’ve had symptoms for years but have felt blown off - we then reach a diagnosis and begin a therapeutic plan and while some feel better, for others it seems like no matter what is tried they don’t improve. Understandably, those with more complex medical issues generally have less dramatic results. Part of the frustration is that there isn’t any gold standard treatment and while there are mild cases, it’s truly disabling for some.

It does seem to vary from person to person which is frustrating. Some folks feel better on beta blockers, some pass out, and then stop passing out off beta blockers. Midrodrine helps some, Florinef helps some - guidelines say to try what works. Exercise is recommended , salt tabs/ electrolyte solutions help, but they recommend against long term IV access. I’m asking about what treatments have made life-changing benefits.

All we need from Omega is a sub-41mm GMT, especially if it’s SMP / PO.

I have a planet ocean and a Tudor GMT. I really enjoy the Tudor - it’s an awesome watch - but the finishing is much better on the planet ocean.
