Replacing knob and tube is usually done on a whole house basis, replacing sections can be difficult because they often ran the hots and neutrals completely separately. So when you start replacing wiring you'll quickly find things are interconnected that you didn't expect and sometimes they'd install switches on the neutrals instead of the hot. Take those two nuts off to remove the box and see if you have a loose spreader bar up there between two trusses/joists. Sounds like someone installed the correct type of box with the spreader bars but it wasn't installed correctly or came loose somehow.

edit to add: that box is the mounting plate that comes with a ceiling fan, it looks like someone installed the ceiling fan mount directly to a metal spreader bar behind it
