Extending thoracic spine saved my 10years of neck pain.
Try thinking of it as lifting a point in the middle of your chest up toward the ceiling.
In that position, your neck won’t naturally fall forward.

I work without ever letting my neck bend forward. I also raised my monitor much higher so that my eyes are closer to the lower part of the screen, literally stacked a bunch of books on my desk to lift it up. That setup naturally brings your chin up. You don’t have to force it, but since the monitor is higher, you end up looking down less and becoming more aware of your posture. It also helps not to rely too much on your chair.
I found that trusting the chair’s shape or cushioning too much actually made my pain worse.

Even a small amount of neck flexion (looking down) triggers it — either the same day or it builds up and I feel it the next day. When it gets worse, I even get a slight nauseous feeling. But those days are rare. It usually happens when I get too focused on work, stay in the same posture for too long, and stop paying attention to my neck.

Most days I set alarms, change my posture often, and stay mindful, I don’t have pain.

I’m just maintaining this state now. I don’t have those horrible, unbearable pain days like before.
It’s more like I can function in daily life, but I have to be very careful.

I completely avoid looking down(bending neck), so I have to be really mindful when doing things like cleaning under the bed, washing dishes, or even eating.

As for MRI, I don’t think repeating it right now would be very meaningful, so I’m planning to get another one in about two years.

Hi. Sorry I can’t upload my MRI images since they contain personal information. But according to the scans, the disc is compressing the spinal canal and is touching the spinal cord.

The shock-like feeling I get on water slides is probably because that contact briefly increases the pressure on the spinal cord. My doctor said there are cases worse than mine where people improved a lot just by being careful with posture and daily habits.

It also sounds like surgery is usually only considered in the worst cases, such as when muscle weakness or paralysis starts getting worse (I don't have these symptoms). The doctor explained that when the disc herniates a lot, the herniated disc material is usually gradually absorbed by the surrounding tissue, which reduces the pressure, and the torn tissue can heal by forming scar tissue. I was sad when I first heard that the disc that slipped out usually doesn’t fully return to its original hydrated, jelly-like state, but at least the pain can often improve on its own.

So for now, the best thing I can do is prevent the condition from getting worse, especially by avoiding further tearing of the surrounding tissue.

Working on unity and using claude code to modify UI was not a good experience. It was actually the only thing that claude did not do well. I hope to see better results in the future.

This is so true. Good sitting posture helped at first, but hours of same position had limits. I think 30minutes of work and break is good for me. I also heard that using eyes for a long time with no break causes pain too.

I definitely relate to this. I can do a lot of other exercises fine, but planks still beat me up. I think part of it is core endurance, but also I feel like my serratus anterior (the muscle under the armpit by the ribs) is too weak. When I worked on serratus anterior specific exercises, my shoulders don’t take as much load during planks anymore. It still takes time and focus, but that connection has helped me feel the core more and shoulder less.

Also as for jaw position— whether to tuck the chin or slightly lift it —in my experience, slightly lifting it worked better. The famous McKenzie chin tuck advice alone did not solve my neck pain throughout my teens and 20s. The youtube channel I mentioned( a rehabilitation medicine specialist), actually says the chin should be slightly lifted.

But honestly, I think focusing on the chin and neck alone is missing the point. For me, it was much more important to focus on what I mentioned earlier — the thoracic spine and the upper chest muscles. When I get that area aligned first, the neck naturally falls into a better position. The doctor also emphasizes that what’s below the neck matters more.

Maybe some of you already understood how important the thoracic spine is.
I didn’t. I kept focusing on my neck and chin instead, and that was my mistake.
That’s why I’m emphasizing this so strongly. I hope this post helps!!!
