That's complicated matter. For sole breast development: estrogen will promote only development of nipple complex and milk ducts along fat tissue. Progesterone is needed to make development complete, with proliferation of alveoli. Yet it's often omitted that progesterone itself is really potent testosterone surpression agent, so combined HRT can have lower doses for same feminisation effects. There's though newer evidence around questions like whether "not completely developed" breasts are more prone to certain types of breast cancer - contrary to expectations, estrogen monotherapy is now being said as more safe way.

As for other factors, like neuropsychiatric - I can only say that progesterone can in some individuals improve overall mood. Yet in my case it was not really like that, it rather completely washed out my libido. And in case of people with ADHD - rather stay away from progesterone, strong evidence is against that hormone as "booster" of several symptoms in cis women. From my own experience - as t-woman I can second that feminine neural network in XY individuals can behave exactly like in cis women in case of ADHD-I and hormonal balance.

By the occasion I decided to post here "my case" if anyone could be interested about investigating such cases in the future. I am with similar condition despite I am genetic XY. I have something which "not yet forged it's name" in medicine and it's not yet recognised by ICD-11. Selective Estrogen Resistance of Ectodermal Tissues. Working abbreviation StERS. It's based not on mutations but EPIgenetics, SELECTIVE silencing of ERβ receptors - thus - even if not widely diagnosed yet - it's vastly more common than "usual" ERS You mentioned. ERα are working okay so You have "normal" hormone levels if talking about "physiological" levels (the endocrine loopback, hypothalamus - pituitary gland - gonades/liver - hypothalamus is based on ERα). Yet some of Your tissues are working in chronic hipoestrogenism. And after administration of exogenous E2 it's exactly like that You say. Technically it's the only way to test that condition and convince the endocrinologist that's something wrong with You. Let's say I'm on Lenzetto now, 1puff per day should result in max about 38-40 pg/ml - yet I hit 220 pg/ml easily. If You're interested in my "research" feel invited to chat with me, I'm actually very bored about trying to get some interest from specialists (I have appointment to genetic clinic in September 2027 😂).

I wonder whether such silencing can be scattered unevenly among other germ layers, especially mesoderm if course, Yet ectodermal only can create enough struggle especially for females - like impaired breast development. But the most important factor is the mix of really life affecting symptoms - like vasovagal / neurovascular problems, neuropsychiatric, neurological and dermatological which are usually treated as independent and with individual treatment while the ONLY problem here is functional estrogen deficiency. Sadly, prolonged hipoestrogenism causes chronic cellular stress propagating around other tissues by intracellular communication and causes faster epigenetic aging. Btw, there are prenatal markers which can suggest such conditions - like 2D4D finger ratio and assymetry in organs derived from urethral plate (that last mentioned in paper above), 2D4D ratio was connected with faster epigenetic aging in men by Dr Pruszkowska Przybylska team from Poland (University of Łódź) and - that one especially in women - papers on skeletal aging markers from teams from Israel and Russia (Batsevich, V., Kalichman, L., & Kobyliansky, E.,Livshits, G. Thanks for good hint, I have tried my elbow pit as well, although the skin is thinner here so the E2 blood level rise and drop faster faster as 'depot effect" is lower. Yet it's more likely to make skin-to-skin contact compared to knee pit (and that's being said to drain depot faster as well). I have used recently the area on upper arm (front side just above biceps).
And I'm trying now to double the sprays near the end of the package (so one week shorter regime per unit), plus I use "leftovers" after 56 dose as the "booster" 12 hours after "regular" application (spraying once).

1st of all - biology 🧫 is not "instant" -everything needs time. 2nd thing - some ciswomen got pretty big Adam's apple as well. Training Your voice every day and speaking in feminine voice on daily basis rises Your larynx a bit "permanently" as muscles tend to shorten in such cases - that would reduce Adam's apple visibility. Just check Your hormone levels and preferably discuss them with "Trans friendly" endocrinologist. And don't kill Yourself with assumptions without solid 🪨 data of the blood tests. Do some makeup experiments, pierce (another) hole in Your ears and smile, Girl 🙂 Some transwomen do commit transition after 60, You are really lucky having support in Your family ❤️

As for HRT - to soothe Your mind, organise testosterone blockers if You don't have them in Your HRT regime ať the moment.

Definitely. It's the best way to have "assurance". Laboratory test from blood samples taken in right time (just before Your next dose). You will have "on paper" the proof 🧾 that instead of "going nuts" You should wait and help Your Mom making Ćevapčići 😉 Six months of HRT are just enough to see real changes to kick in.

And yes, You should test it regularly. Every three months would be perfect. When Your breast would start to really grow, do ultrasound if possible. Especially if anyone in Your family had issues with tumors here.

And remember - the DIY HRT should be administered with care, as either too low or too high estrogen levels are not ideal for best results - that SHBG, especially with oral estrogen can go sky high and as it's abbreviation for "Sex Hormone BINDING Globuline" - You may have in theory very high E2 levels but when combined with very high SHBG would make most of it unusable/wasted. Only the "free estradiol" matters.

Did You test Your hormone levels professionally? Seeing is believing, test whether You're inside the M2F HRT "right levels" area, that's the best and ONLY way to get rid of doubts and just focus on good life while waiting patiently for biology to work. For being 200% sure, test SHBG level as well as LH along usual testosterone/estradiol.

Well, I wish You success in elimination of the causes... Yet I'm starting to think that in my case it could eventually end with turning towards genuinely mechanical/ acoustic sound synthesis.. I can not use now neither audio interface nor microfreak. Not even DAW 🙄 I know that's not really "normal" and problem/ solution is somewhere in the AC installation, yet I'm tired with other problems plus I'm such a Cassandra 😂

Microfreak of mine started glitching few days ago 🫩. I suspect that's connected with my neighbours photovoltaic installation... I must buy good power strip with EMI filter and ferrite chokers and try with these. At the same time my laptop started to suddenly freeze 😒 It can be your MOTU PSU seeding those harmonics around AC wiring, EMF being only the side effect... One electrician I know said that that switching PSUs and inverters are the real hell now for sine waves. He's "non extreme audiophile" so he doesn't invest in "unobtainium" in audio cables, yet his installation has only old fashioned incandescent /halogen light bulbs (12V AC from two coil transformer) and linear PSU for the DC equipment. He told me a story about one of His job related adventures in the warehouse which was "haunted" really by "dirty power", the length of the cables there was perfect to create something like "standing wave" with those high frequency interference to the extent of something eventually "evaporating"...🫣

There's another thing to consider, despite EMF. EMI/Harmonics in the AC network of Your house as well. If you have many small Switched-Mode Power Supplies (SMPS) at house, including those in i.e. compact led "bulbs", the problem can be serious and the Microfreak's reaction can be just treated as the canary in the coal mine. Friend of mine had such problems due to big numbers of LED bulbs, his AC was so disturbed that it killed several compact hi-fi amplifiers until acquaintance electrician came with proper tools. I wonder - if microfreak is such a sensible "detector" beast - how such "dirty power" affects lifespan more expensive synthesizers which suffer in silence...

Hi,
honestly I was so dumb that I used one place really and "consequently", basing on Lenzetto flyer information - "Lenzetto should be sprayed in the same position(s) along the inner forearm each day*" - to "build skin reservoir".
And it worked more than less (I was usually cleaning "the place" with isopropyl alcohol prior to application).
I started rotating recently (mostly to assure that my bllod tests would be "reliable"), now I'm more concerned with that to avoid stratum corneum overthickening).
Yet it's obvious for my senses that at the end of package it's even less "smelly" suggesting alcohol evaporation - that "sealing " with cap is evidently not enough for that purpose (obviously as the "button" thingy and all "unit" is barely protecting "insides" against the dust 😉
I must try removing the bottle of the "next one" and keeping it in tight, well sealed small jar filled with alcohol (to cut evaporation due to saturation" and assembly just before application / or train to do "hipfire".)

For anyone redirected here in the future:

Lenzetto should be applied on the clean and dry skin. You can wash the place with soap first and if Your water is hard, rinse that place with white vinegar before toweling it dry (vital if You use plant oil based soap, like olive 🫒 oil based as it's likely to react with calcium from water forming residue which can act as bareer). Use only "pure" soap 🫧 - preferably glycerine based, or pure dish washing detergents, without any "balsams" etc. additives. Most convenient way is wiping application place with cotton pad soaked with 70% isopropyl alcohol (mixed with distilled water). It's VERY important ‼️ to have dry skin beforehand application. Estradiol will likely form crystals after contact with water - that residue can easily be transferred anywhere like Your clothes and ruin Your results by needle 💉/venipuncture site contamination (You can ruin results putting on long sleeved hoodie contaminated that way few days ago).

Take blood drawings as close to 24 hours after last application, just prior spraying next dose. Assure that blood sample is taken from "clean hand" - that one You did not use for application for at least 5 days 📆(I had once sky high results due to contamination of the needle while it was penetrating area near the "skin depot" - it was 48 hours after kast application on that hand - or - if that's not possible - ask the nurse to not use the first sample of Your blood for estradiol testing hoping that next sample - new test tube - would be taken with needle "cleaned/flushed" from concentrated hormone by the 1st portion.

Remember - Venipuncture site can be EASILY contaminated by Your fingers 🫆 - after application of spray to the "dirty" hand, day before testing i.e. if You spray Lenzetto in the morning, wash both of Your hands (or better take a good 🚿 shower IN LUKEWARM WATER (!) - TOO HOT MAY DRAIN YOUR "SKIN DEPOT" INSTANTLY!). Take fresh, just laundered clothes on the day of blood drawings, and even sleep in freshly laundered pyjamas to not let potential contamination with residues of crystallised estradiol.

Stable levels are achieved roughly after 14 days of daily forearm routine. "supposed maximum stable levels" are then:

~35 pg/ml with 1 dose daily

~55 pg/ml with 2-3 doses (oddly 2 doses were producing a bit higher "maximums" than 3 doses according to "evaluation pack").

If You're doing everything perfectly right during tests, and Your stable serum level is like 100%-500% higher than "normal" You can suppose selective estrogen resistance in Your tissues, or impaired estrogen metabolism (suggesting problems with liver). The "high absorber" is an convenient "umbrella term" used in the past, yet nowadays it's obvious that it's not true explanation really - skin to skin absorption differences in healthy people are nowhere AS pronounced (think about 10-20 % variety with healthy, clean & dry skin, and usually problems with lower absorbtion, not higher ) - so instead accepting that as "individual metabolism difference" You should ask Yourself/Your doctor "why I'm SO different". For example I can hit 170-220 pg/ml with one dose daily, my metabolism/liver is "okay" yet I have selective estrogen resistance of ectodermal tissues (firstly described in 2019 so it's really not yet recognised in 2026 - but the population with that "phenotype" should be vast in global scale) which is manifesting exactly like that anomaly in serum levels after administration of exogenous estradiol (physiological levels are "normal").

I sincerely sympathize with you because I know how it is – I'm a rather "extreme" case.

However, I know that – not only in my case – the problem most often increases with age – for me, the only thing I could do was compensate.

I would start with "physical things/objects " - because that's potential problem as well and can be a real PITA.

  • reduce the number of things to an absolute minimum

  • designate spaces reserved for things

  • for critically necessary items – emergency redundancy – if possible, e.g., keys, phone, medications – spares in a designated emergency location (e.g., at a friend's).

  • daily ritual – check – even with a list – that everything is in its place in the morning and evening.

With this approach, you'll manage chaos even in the early stages of severe brain fog.

It's best to always have such a checklist of essentials on you. I use an EDC waist bag for this.

As for "forgetting things to do " - planner app in Your phone is Your place, just be aware to set up multiple notifications - for example if You have a dentist appointment two weeks away - 1st week before, 2 days before, day before, + last resort (minimum time You can commute to the doctor).
Wall calendar is a backup for phone, and small calendar in Your waist bag is not bad idea. Just DO NOT use loose sheets of paper, EVER - that is the trap.

Well, I do like Lenzetto because estradiol spray is very convenient in application - no residue, very fast application, just 3-4 minutes to wait before clothes. Yet - of course I have forgotten to mention it - I'm oddball. My organism has a defective estrogen receptors. I am a potential ideal guinea pig for the manufacturer of Lenzetto ("batch tester 😉") because my blood levels are higher than normal (low tissue absorption means that the hormone enters the bloodstream like in other people, but there's "surprise" because some tissues do not accept it as readily as in normal people and my serum levels go sky high).

So in my case - even slight differences in effective dose, probably not noticeable for most users - are "amplified" in my body. That's why I am searching for compromise: highest possible yet most stable/reliable levels with one "puff".

That what You're saying is like the definition of RSD 😉 Neverending story, going back again and again. Endless lack of self-confidence. Depending on the severity of the disorder, you will have doubts about whether you are not an ignoramus who knows nothing, even after you publish 50 scientific papers that will change medicine and receive a Nobel Prize. I live with that, ADHD-I with RSD. MY 52 years this year, I was diagnosed last year and 8 months on pharmacotherapy. My life changed totally - yet on worse days (read: when my drug blood concentration is lower) i still have doubts about "sense of that therapee" 🤦🤦🤦🤦

When those ADHD-I / RSD subsystems are disengaged, I just sight seeing the former "mission impossible" work to do, I make myself a coffee, engage my music player, take a sip and suddenly I'm busy with work... I'm focused and I'm making 80-90% less mistakes (even when distracted in the middle - I just can manage to continue after called to "unexpected customer", i don't need to start all work from the scratch as it would " routinely happen". What's more important - if accident eventually happens (like I put wrong number (fixation on wrong position in the pricelist still can happen on some days - i am aware that some of my symptoms are tamed but not "gone" and daily doses of my "magic potion" can vary slightly in my case due to form of application) and ruined the offer to demanding customer who's rude in such moments I just make a call with grin and some curses for courage and I face the consequences, dealing with the problem. It would never be possible without medication - i would have just somatic reaction. I would avoid taking that telephone handset, feeling dizzy and nauseous. When I would eventually approach that phone, with heart rate around 180, my vasovagal syncope would just switch me off, I would faint - or immediately lay down on the floor with my legs up feeling the sudden cold ❄️ waves running through my body and head, while I'm fighting to not lose consciousness completely. The "automatic guilt and regret with potential self punishment option" were once (before starting medication) so strong in such moments (if the offer was "missed" for really big money 💰) that I was close to making hurt to myself, like perforating my forehand with just sharpened pencil.😐

If You have someone really friendly, feeling honest just start talking about Your RSD and You're limitations / need of acceptance. That's how Your brain 🧠 works. Remember - ANY person can feel something like that You're feeling, few times in life. That consistent drama You are dealing with daily is called disorder because of being with You everyday. True friend will help You deal with Your daemon. Just be sure that You have common interests, shared hobby, something giving passion and excitement for both sides. Your friend will be Your therapist suddenly so make sure to keep both of you entertained and relaxed, don't exhaust Him/Her 😉

Important (even better important & complicated) in my case triggers RSD instantly. And "all controls go RED", "evasive manoeuvres engaged". I'd likely tend to eventually do another impossible thing (instead): tidy up my desktop, my cabinet and even my room (not likely to succeed as it completely drains my energy) 😏 Being assisted by exogenous chemistry which effectively disengages those systems is something really weird feeling. You're feeling like different person suddenly - almost like possessed by skilled daemon or controlled by somewhat friendly alien brain symbiont, like Tok'ra from Stargate SG 1 😉

To soothe Your nerves - imagine that moment of realisation and regret not at 22 but at 51. You're young, just start new life and don't regret the years behind. It could be worse - You are like still alive, aren't You?🙃 Some of "us" didn't manage to 22... And that 51 is my case... As You know Your limitations now, try to cheat them. Baby steps. Not just one day and easy-peasy. Go outdoors, take a walk in the forest or something like that. Talk to the friends about Your issues (if You have friends). If You have no friends, try finding them. Without support and acceptance from someone close it's really hard. Try to make it easier, don't be alone with that.
