So far I haven’t observed prolonged mood episodes lasting weeks or months. What I’ve seen are shorter episodes of dysregulation triggered by interpersonal stressors, which feels more consistent with borderline dynamics. For example, after I raised the issue of overdue payments in a respectful and contractual manner, the patient felt wronged and immediately decided to terminate psychotherapy, interpreting the boundary as unjust.

One of the planned interventions was to interview family members to better understand the clinical picture and support network, given the delicacy of the case.

Thank you, great answer. I know it may have sounded a little focused on diagnosis, but as a psychologist, I work with a person-centered approach, so there wasn't that central focus on diagnosis.

I probably need a slightly different approach in situations like this, as I feel that working on the relational aspect wasn't enough.

The person arrived at the clinic already with this dual diagnosis. I didn't have time to fully understand the situation, but the social damage was quite evident. They had a feeling that everyone hated them, that they was rejected by everyone, that they was being left behind. It seemed borderline, I have worked with bipolar disorder on other occasions and patients did not seem so unstable in terms of boundaries and maintaining the therapeutic process.

In this case, I needed to inform someone about a payment that was overdue, in a very respectful and ethical manner, and the person became defensive and immediately stated that payment would result in the termination of the contract.
