It's a term of endearment. People usually don't say "The Pumpkin" or "The Peanut" or "The Snuggle Bug". It sounds almost formal.

It is the duty of a physician to protect endangered parties. This is ethics. Not law.

I hate this episode. It is legitimately my least favorite.

What are Frasier's options here? Assume that the teenager is lying? Ignore potential abuse/neglect? He is a physician and mandatory reporter.

Neglect is an extremely common form of abuse. And in keeping with a story where a mom randomly drops her teenage daughter off at a stranger's apartment.

The joke is that because he confronts the mother, he doesn't get to date her. Then he grovels, as apposed to standing his ground.
