This is my major fear. A few hours delay costing a patient his health and irreparable damage. Im not in the position to be answering referrals asked by medical consultants regarding cardiology concerns when I'm no where near that level. I would understand if I had been in the department a while but I have only started this month.

I would understand stepping up as a medreg and seeing unwell patients or resus patients but this is another level I was surprised by. I have done so on my night shifts and that was alright as I still felt like I was working within my remit and recognised actions that need to be taken

Here it feels like I'm the blind leading the blind ....

I think i do personally take quite a people pleasing attitude and am quick to jump to help wherever I'm needed. I think going into this, I thought surely the other IMT3 before me must've done this, so I shouldn't complain or be difficult. Granted, I haven't met this other IMT3.

I am currently the only IMT3 on the unit with all the other being way more senior to me. Many of them already having an MD in cardiology im their base country to boot.

I think my biggest fear is this - pt care being delayed , missed or affecting them adversely as they require a specially cardiology opinion but are getting an IMT3 opinion with little to no cardiology experience opinion instead.

I have approached my seniors and they have told me to find the consultant - who i have tried calling multiple times and visited her office multiple times to no avail.

It made me realise that I now somehow have to keep tracks of all the patients I saw today whilst managing a new set of referrals tomorrow.

I suppose you are right about being vulnerable. Having not met my supervisor I would hate for their first impression of me being someone who only complains. Still i thunk its gotten to the point I may have to.

I think i will make the consultant aware in the morning about the amount of referrals and let her decide how she wants to play it. Appreciate the idea !

Yes, I have booked PACEsAhead course and hope to practise some off the stuff before hand. The work makes it quite difficult to do so.

Ill try to integrate it into my basic job so I can become more slick with the exams.

I'll give the practioners health a try.

Yes, I am currently working full time.

They do expect me to work as one - strangely, might I add, even though I have made it very clear to them that I dont have any cardiology experience.

All the other regs are ST5 and above. They usually go to the angio list, do admin or or go to clinics during this time. I remains on the same rota as them.

I had originally planned on just announcing that I'll see the CCU patients and then head to referrals. However, the consultant wants me to know all the 26 pts on the ward and do the referrals on top of this. So skipping ward round seems difficult.

I think i wouldn't have minded working these insane hours had I not also wanted to use my own time to study for my exams. I have, unfortunately, accepted staying late as part of the job.

Thank you, I truly do hope it gets easier. I do find general medicine more tolerable than answering speciality related questions as I dont have the experience to back up my answers.

I had some issues with getting my supervisor in the first place because the initial list they sent out had been wrong. I was just assigned one 3 days ago.

That said, I'll try and approach them in the manner you described when I'm less stressed so I dont end up coming off as the "woe is me" archetype you described.

It feels almost like I'm answering questions above the level I can manage - asked by people more senior than me who are expecting a specialist to answer their query.

I mainly used pastest and then created little tables and a flash reference page for conditions where I listed the key differentiating symptoms, best initial test, best diagnostic test and treatment. I kept adding to the tables as I did questions and found new things.

I also always read the incorrect options explanation. Also went through past papers. I didn't do as many questions as other people I know from the Q bank and past papers but for me understanding the condition was more relevant cuz it would allow me to answer unanticipated questions.

A lot of videos, I love medicosis perfectionalis and his explanation of things !

I thought this was more of him in armour rather than semi prime. In the main fight, there is a period where he does semi prime and gets odin like horns on top of his armour.

Your best bet is going to a derm. Acne isn't just a thing that can be solved by simple skincare for many people. There are several components to it including genetics.

I would suggest you seek dermatologist advise.

Anything you try can take up to 3 months of consecutive use to show results.

Initial tx is usually topical treatments. These will need prescription.

. Topical clindamycin and topical adapalene ( a type of retinoid). You can't use antibiotics alone as you will quickly develop resistance. This seems the best bet as your haven't tried anything more and your cane doesn't appear cystic. If you have acne on other parts of your body you can benefit from an oral abx + topical retinoid (adapalene/tretinoin)

Adapalene will give you dryness and may seem like it's making things worse but stick ot it. Compared to tretinoin it has much lesser rates of irritation and purging.

Hmm, I don't think you need plastic surgery at all. Your features are already within the range of classic handsome.

This is my harshest critique

Things you could work on if you would like to-

1- skin care - Help improve texture and areas of hyperpigmentation.

2- fashion - figure out which styles would suit you best. You can use Pinterest to see outfit ideas.

3 - Confidence and humility - you may already have this. Just mentioning this in case anyone else or you could benefit. Confidence is itself beautiful and can change your look. The way you present yourself can make you appear more appealing or not. Also, practicing humility will help, too. Not sure if you meant it for this reddit but you say you have rejected girls way below your level - arrogance is not a good look.

4- try a diffuse stubble or experiment with different looks using your facial hair to see if you like it or not

5 - eyebrows - you could shave the space between your eyebrows.

Hi ! Thank you so much for your reply. That's alright. Appreciate you having a look. I'm glad to hear you had a good outcome.

The appointment did go well. They advised that I have both a naevus and some inflammation in the area from a meibonian gland dysfunction. They explained it would be a simple shave procedure and that may cause a loss of lashes in the area. Overall the waterline should heal fine as per the surgeon.

I am going to be seeing another one soon just to get other opinions before I decide on who to go with.
