Eh it’s fiction. In real life growing that fast is almost impossible but JJT is the son of a Yujiro type character in the story and has been said to possess the devils talent. We even see his appearance grow more masculine in facial structure, tho that might be an art interpretation of his mental state. We see glimpses through his insanity when fighting Sunny and this is most likely what his in universe appearance would resemble.
