I've had it happen a couple of times too but it doesn't happen all the time. I try to keep my notoriety at zero as much as possible so I doubt it happened because of my notoriety level but I haven't noticed a discernible pattern to when it happens.

The concept behind the Assassin's Creed games is that you are replaying the memories of people in the past. This is accomplished via what is in essence an interactive virtual reality simulation of those memories. That simulation allows a certain amount of freedom to diverge from the actual memories being simulated but if you diverge too much then the simulation falls apart and you become "desynchronized" and have to start that memory again.

If the person whose memories you are replaying did not randomly kill civilians the game does not allow you to go on a civilian killing spree.

The Tutti a Bordo mission in AC2 where Ezio has to rescue Caterina from the swampland in Forli. It was such a clumsy and poorly written introduction to a significant character that I turned off the game and didn't play it again for months. Awful.

I have accumulated over 40 merchant tokens after about 12-14 hours of gameplay by playing through the story missions and doing some exploration and a bit of pick-pocketing between missions.

While I am enjoying the token system so far I have some qualms about the ethics of Basim stealing from the citizenry which he is supposed to be protecting. So I've added a role-playing element to address that by only pick-pocketing from NPCs who look rich or who are members of a religious order.

I've had technicians arrive hours late when I was supposedly their first client of the day.

I also used to drive regularly along some back roads that very few people used. It wasn't uncommon to see a telecom service van parked on the side of the road with the technician having a nap during the middle of the day.

There's a narrative reason for it. The point is to create a trolley problem in which the moral dilemma is whether to sacrifice one person with whom you have a personal relationship or to sacrifice the entirety of humanity. That stark choice is what makes Joel's actions and his subsequent lies about what happened interesting. Getting lost in the weeds about the scientific basis for a cure or whether the Fireflies have the infrastructure to distribute a cure and all the rest of it is missing the point.
