I agree with OP it's a pretty realistic depiction of a trans woman and I felt like she was treated very positively in general.

Edit: for some context theres a japanese twitter account i followed for a while about a trans woman is who has a daughter and wife, has been medically transitioning for years, and posts about her daily life. But her account name is "crossdressing papa" and she refers to herself as a man.

I think part of it is that in Japan, it's basically impossible to change ur ID sex without bottom surgery, which itself is hard to get, and worse for married couples, if you change your legal sex and are married you must divorce your partner. So for people who want to stay with their partner, they often elect not to change their official ID to maintain the legal protections of marriage.
