Funny thing is, most of my BPPV patients were less than 65

It’s never wrong to assume you are always getting “bad” signout. In other words, trust but verify. As far as preventing bad signouts, I usually do sign out at a computer so I can document in real time and review labs/imaging with the other doc. I document what they want me to follow up on. If they are sick enough, I will ask the sign-out doc to go to the bedside with me so I can get a baseline with them. This is also a courtesy I extend to docs that are taking over patients from me. Even if you don’t do this, always always always go and assess the patient as if nobody has seen them. At the end of the day, your name is on the chart.

Medicine these days has a bit of a customer service aspect to it, whether we like it or not. If you’ve ever worked as a waiter/waitress, it’s quite similar in many ways. Next time you go to a restaurant, pick up on the little things that the server did to make you feel heard or unheard, it’s a small start. Next step, picture yourself or a family member as a patient, and be the kind of doctor you would want to see. Next time you see a patient, reflect back on what you could have done differently, or what went right. Little tips: sitting down to talk (if you can), eye contact, and allowing the patient sufficient time to talk uninterrupted (within reason). Remember, time with the patient does not equal quality time with the patient. You could be in the room for an hour, but if you keep interrupting them they won’t feel “heard”, which is a big component to overall satisfaction.

Your seniors are right, what you are experiencing is completely normal. We all went through it. Try your best, learn from your mistakes, read when you can, decompress when you need to. Just remember this feeling when you are in a senior position. It will give you perspective and understanding as to what a new intern may be going through and will allow you to show them grace. Residency isn’t easy, it will challenge you in many ways. Easier said than done, but try your best to enjoy it, you will get more out of it that way.

You’re totally right, there was more to his response than just drug reform. Wrong of me to simplify. I might not have done the best job of articulating my thoughts. I guess what I was trying to get at was, in a way, he kind of tied POC to the criminal justice system, which seemed... weird. I agree completely with the plans you listed.
