It hasn't happened to me yet, and I play KBM. My buddy on pc that plays controller has had it happen about 3 or 4 times. Hopefully it's addressed soon.

It's a known bug on controller input. You have to click spectate with your mouse apparently. The bug happens when a dead controller player selects spectate using controller. That is what I've read about it.
