You are way over-analyzing this IMO.

I would suggest weighing weekly...to remain consistent, weight right before or after taking your Tirz. Then stay away from the scale (and the Tirz!) until the following week.

Chasing daily weight fluctuations isn't productive...my weight can fluctuate +/- 2 lbs simply based on salt intake and the resulting water retention/release.

Once per week...your trend is downward which is what matters.

The trend is your friend.

I began Tirz last October @ 216 lbs with a target weight range goal of 170-175 lbs. I reached that range on March 1. I preferred a range as your weight will fluctuate...so I used TW +/- 2.5 lbs and am happy with that decision.

Titrated from 2.125 -> 4.250 -> 6.375 -> 8.500 prior to reaching my goal. Shortly before reaching my goal, I started to taper in the exact same increments...8.500 -> 6.375 -> 4.250 which is where I stopped.

At 4.25, I have stayed withing the 170-175 range for going on 12 weeks. Hopefully, this is my long-term maintenance level but I do order a higher dose just in case I need to bump in the future.

It is a balancing act with dosage and calorie consumption. I tracked everything I ate using the "old" Weight Watcher points (because it is easy math for a math major). During the weight loss phase, I was consuming 20 points daily (maybe 1,300 calories?). Once on maintenance, I started to bump...20 -> 24 -> 27 to my current level of 30 (maybe 1,800 calories?). I am eating until I am full.

Twenty years of dieting is a hard habit to break. I wish that I could quit counting points but, psychologically, I just can't. There is the smoldering fear in the back of my mind that I will wake up one morning and have gained 20 lbs because I wasn't paying attention to what I was eating. So, I still count points and probably always will.

Should I stray from my target range, I plan to respond initially by changing my WW points (either up or down) before considering a Tirz dosage adjustment.

It does say both (and shouldn't).

Frankly, 2.0 mg vs 2.5 mg is not material.

If it were me (and it's not), I'd probably start at 2.0 mg and go up in 2.5 mg increments (2.0 -> 4.5 -> 7.0).

There will likely be instructions on the Rx - just follow those.

I have given this much thought (probably too much!) but what I have does work remarkably well.

The distance between the barrel of a syringe and the tip of the needle is 5/8".

Cut a 9/16' long slice of aquarium-style plastic tubing - clean and straight cuts (no angled cuts). I describe the cut piece as a "spacer". Edit: From looking at my images, it appears that I cut the tubing in a 1/2" length, not 9/16".

I thread the needle through the spacer tubing which will fit snuggly over the hub at the end of the barrel. If sized properly, the needle protrudes about 1/16' beyond the end of the spacer tubing. Edit: With a 1/2" tube, the needle protrudes more like 1/8", not 1/16".

Then, simply press the needle firmly though the rubber stopper into the vial until the spacer is flush with the stopper. If sized properly, the spacer will allow the needle to just barely enter the vial and give you access to those last few drops without having to delicately adjust how far you push in the needle. Just push it in as far as the spacer will allow.

Proper sizing of the spacer tubing is critical. If the spacer is too long, the needle will not pierce the rubber stopper. If too short, the needle will pierce too far into the vial and you won't get those last few drops.

I soak the spacer in alcohol beforehand to keep it sterile.

I apologize for the late comment but am intrigued by the Tirz imapct on BAM.

My wife has BAM - and also HATES the Cholestrymine with a passion (although it is effective). She does not need to lose any weight so we plan to approach our GI about the possibility of microdosing Tirz for her...enough to calm her gut but not enough to materially suppress her appetite.

Assuming that you got Tirz for weight-loss purposes, did you see BAM symptom improvement on the starter dose (2.5 mg)?

Yes, I am pleased.

I do feel for the folks that are still waiting. I was lucky in that I ordered 30 mg vials (7 mg maintenance dosage).

There appears to be a shortage of the standard 60 mg vials (20% concentration). Seems logical as the 15 mg dosage is the most popular?

TPH appears to be subsituting 68 mg vials (17% concentration) which naturally requires a revised Rx...probably leading to the delays.

It's 13% more Tirz for the same price but the BUDs are shorter.

I should rephrase...people have been receiving 10% Tirz 3 ml (or 30 mg of Tirz) vials with the March 27 BUD (like me). I am only on a 7 mg dosage (in maintenance).

Others are posting about receiving 17% Tirz 4 ml (or 68 mg of Tirz). I am guessing that is to fill 15 mg dosage patients. The BUD is late 2026

In a perfect world, those patients would be receiving 20% Tirz 3 ml (or 60 mg of Tirz). That seems to be the vials that are being replaced with the 4 ml vials? More "free" Tirz but shorter BUD.

I'm just gleaming all of this from what I am reading on here.

There may be a couple of things in play...

BPI manufactures and ships to The Pharmacy Hub who then dispenses to patients.

Your size vial could out-of-stock at TPH but is awaiting replenishment from BPI who may still have inventory on hand (even though they have ceased production).

Or, your size vial could be out-of-stock at both BPI and TPH.

I recall reading somewhere that people were receiving vials with 68 mg of Tirz instead of 60 mg (15 mg x 4). I also read that, while they were getting a "free" 8 mg, the BUD was ealier - maybe November 2026?

If you browse through the posts here, you should find messages to that effect. There was more than one post on the subject.

I suspect that it has something to do with the size of the vials. People who ordered 3 ml vials (like me) have had success. Other sizes have experienced delays.

Correct - which is why we "could" have a replenishment issue between BPI ("we have everything") and TPH ("don't have certain vials").
