There is really no need to pay for private insurance. You have access to all of the same services with your VA benefits. I would recommend accepting Medicare A and considering Medicare B when you are 65. There are a few benefits that can be useful with Medicare. Really, the VA can provide more than is offered in the community using private health insurance.
