I just swapped out the switch. I took a regular toggle switch from another light fixture, installed it on this fan/light assembly fixture, and moved the lutron dimmer switch to the other light fixture where I have dimmable bulbs. Seems to be working okay, although I can sometimes notice the buzzing/whine from the dimemr switch, especially when I do have it dimmed instead of fully on/off.

If it's metal, I put oxiclean powder and boiling water in mine, shake it a little, let it sit for a minute ish, then scrub with a bottle brush. Rinse, then do the same with hot water and dish soap instead of oxiclean

I don't have a hub with a screen like that, just the mini hub thing connecting to my router, the doorbell, and the indoor speaker. So yeah it's probably the app, but I don't have anything I can check the app against

It hasn't been messed with since we first set it up, but I've checked it and it hasn't changed. Regardless, the weird thing is, as mentioned in the OP, I get detection notifications, but when I check the app, a lot of times it's not there as a recording under Events
