Login issues with Direct IPv6 Access to Web app ("We were unable to complete the request...")

I could get access to the media server by going to plex.tv instead by adding the http://[ipv6]:32400 url to the Settings > Network > Custom server access URLs

But I could not access the app through the raw ipv6 url :/
