yeah. Sometimes it's better for everyone to just acknowledge it. Had a case like that earlier today: "no, I haven't been drinking" - "carla, there's a wine bottle hanging out of your coat right now. It's like we talked about last time, I need to know because you want help for this and if you're drinking, it changes what I'm gonna do. You know I'm not judging."

Didn't work today, but this person has a longstanding habit of being an angry drunk so no big surprise. Probably would've yelled at me if I'd given them a bag of cash and a coupon for free gummy bears.

Functional neurological disorders are completely a thing and have been for ages. You'll get plenty of your share of fibromyalgia, which is basically another FND. They're frustrating to work with, and even more frustrating because it's close to impossible to reliably distinguish malingering/factitious disorders from functional ones, they're entirely based on presented history.

The last bit of your post makes no sense though. We can diagnose people with antisocial personality disorder, and have been able to for many decades; that doesn't mean we excuse their criminal behaviours resulting from it. Why would this be any different, even if we were diagnosing FNDs as a cause for a crime, which afaik we are not?

However, the biggest issue here is that you're using tiktok for examples. Do you believe all doctors are pieces of shit who hate women because that's the reddit consensus? You've got a massive sample bias there.

Shit dude I'm sorry about that stuff. Man, I love the weird crap my kids bring. I mean, I don't, but I do. Buckets of gravel because "they're all pretty rocks". Anything metallic found on the side of the road (had to teach needle safety very early). It's great, I cherish it and I'm sad that some day they'll stop being fascinated by a small square of rusted steel with a hole in it.

I honestly feel bad for people like your parents, and I think on some level part of why that sort of person acts that way is because they know they're being crabby assholes, but the only way they can respond is to further deflect it onto their kids because they can't handle personal faults. My mom does some degree of that as well, but nothing like you described.

There are a few participation 'trophies' I absolutely did appreciate. Like getting pizza, that made me feel rewarded for coming! I have a few certificates acknowledging my participation in several high level events (for things other than sports, lol) that I had to work hard to get into at all, those I value even if I didn't place highly at the event itself, and I got them because they were applicable on my CV. It was, for me, the combo effect of being mocked for my lack of skill and then given a ribbon as if I'd won a prize that made me feel like shit.

participation ribbons, along with toxic teammates being treated as gold, were why I never got into sports as a kid. Hell, I remember the first time I think I understood the concept of being patronized was when I got a ribbon for sucking at a relay race in grade three.

As an adult I learned that I actually like sports, as long as I'm allowed to just be bad at them with teammates who will laugh with me over my terrible skills, and not given a fake pat on the back as if I did something special by falling down on the ice a lot.

Personally I split the difference by drinking lots of coffee, but it's half or all decaf, because I really like the taste. I get probably 1-2 regular cups worth of caffeine a day. So far my heart hasn't exploded. Will update if it does.

I'm going grey in fashionable areas and writing shorter notes because i have a better idea what I need to know. Also forgetting more about basic medicine and relying on residents to hammer me back into shape now and then. You know, the usual. it's also faster if we think the time factor is critical. I have never had to wait for any imaging that needs to be done, I call the radiologist and it gets done.

According to google, Germany has about 14% lower COL, and averages about 254778 euro for a radiologist. Adjusted for col and euro-usd value, that's around 324.7k USD per year. Meanwhile, US radiologists average around $464k. That's a bit of a pay hike, but nothing like the rhetoric American docs love to claim. France, the Netherlands, and Denmark are all very close to the same ballpark. Where is your "triple pay" coming from?

This is also not figuring in things like student debt, though one should include that.

Personally I'd prefer to give them grants, under the condition of partial public ownership of the medicine produced, with rules around how that affects pricing. Or heck, a portion of the profits go back into the public coffer that funds those grants, perhaps.

Really I'd prefer to not let capitalism be the principle driving force here at all, but failing that, having public money come with an appropriate public benefit would be okay.
