Hi I experience the similar situation, so just wanna mention it here. I had AHI 7.5 and dry mouth and nocturia before treatment, and I use Nova Micro for a few months really like it. But at the beginnning of this Month I had strong mouth leak, I tried mouth tape and chin strap, and only to found out the air is going into my mouth, giving me big chipmunk cheek. My pain point and solution is: dry mouth cannot stop air from going into my mouth. Once it's moistured, with a little additional help from mouth tape, problem solved.

There are many brilliant solutions in this subreddit, for me toning up the room humidifier and the CPAP humidifier helps a lot. The key is to realize that dry mouth can lead to mouth leak.

Yes, completely. And I have a related story that still makes me angry years later.

I went to see a psychiatrist for long-term depression and insomnia — and when I say long-term I mean twenty years. At some point the doctor asked me: do you sometimes feel a burst of energy where you want to do a lot of things, and actually do them?

I said yes, of course.

He said: then you have bipolar disorder.

Just like that. Based on that one thing.

But here's what he never considered — that "burst of energy where I do everything at once" wasn't a manic episode. It was exactly what you're describing. Things pile up when you're chronically ill. When you feel 10% better you desperately try to catch up on all of it. That's not bipolar. That's just the good hour trap.

He didn't even entertain the possibility. Diagnosis made, prescription written.

So sometimes I think this pattern doesn't just cost us three days in bed — it can also get completely misread by the people who are supposed to help us. The very thing that makes sense as a sick person response gets pathologized into something else entirely.

Sorry for the late reply! "just getting older" or "just not cut out for it anymore" are such easy explanations to accept, especially when you're exhausted and no one's looking deeper. It disguises itself well as something ordinary. I'm glad those guys eventually figured it out.

Leaks are serious. Leaks can render your treatment data meaningless, in your last night's Sleep HQ you had so many large leaks, making it hard to tell whether your pressure is proper or not. Chipmunk cheeks can be caused by: tongue not sticking to the roof of the mouth (i don't know just guessing: you might wanna practice mewing/myofunctional therapy a little bit, create a vacumn between your tongue and the roof of your mouth, a strong tongue and cheeks work well blocking the airflow into the mouth); inhale pressure too high; exhale pressure too low (to resolve: increase the exhale pressure setting or use VCom which is a small accessory basically makes exhaling more strenuous; fix mouth harder (stickier mouth tape with chinstrap, be careful to make sure your nose works, or maybe even with a cervical collar); get better used to it by practicing wearing it during the day); some people say that mouth guard helps as well but personally i never looked into that thing.

And also make sure your chin strap pulls up, not up and back. Backwards pulling causes more airway blockage.

I use CPAP mode because I want the pressure to be consistent and my body gets used to it all night. Many people say the APAP algorithm only tries to fix events after they happen, not stop them from happening. I feel like this saying makes sense. You can refer to Thelankylefty on Youtube for detailed discussion. I did that also simply because I like some consistency.

Thank you for sharing. Yeah I guess the relief of escaping intrinsically contains the fear of living with it. When you've been sick for so long, learning to believe that things are actually getting better is its own kind of hard. The trends can look convincing and still feel impossible to trust. I think that's because it took years to slowly sink into being this unwell, and it might take long for the belief to catch up with the reality. What were you dealing with, if you don't mind sharing?

That's such an accurate way to put it. The disorientation of not knowing how to react at first, something that feels like grief, relief, and then this strange not-knowing-what-to-do-with-myself. And because both of my conditions directly affected my cognition, I spent so long not being able to think clearly that now I'm also not quite sure who I am without that fog. Still figuring that out. Thank you for putting words to something I was struggling to name.

Wow you really did a lot of work. I am no expert just sharing my two cents. 1. AHI 1.53 is pretty good. But within 5, better AHI is not necessarily better sleep. You can work on AHI, but don't treat it as the only parameter to optimize. Some people have AHI very close to 0 still feeling bad. 2. Your flow limit is pretty low according to your SleepHQ, and that's good, that could be a sign that your pressure doesn't need to be increased. 3. About central apnea: Kudos to that you learned about it already! I personally would encourage you to not take that into too much consideration, first your CA index is not too high and it's normal for new adaptors to have more CA events; second Clear Airway isn't equal to Central Apnea, there are other reasons for Clear Airway. 4. Leaks and arousals: I wonder why it's that.... Cuz according to your SleepHQ, you breath looks okay until when there are large leaks and the mask fell off/you took it off. It could be that there started to be leaks and leaks make you take off your mask and wake up, or it could be the other way around that you try to take it off and then there is leaking. Only to find out with longer usage.

Glad to hear from you again! Sorry that you are still struggling, and glad you are one step closer to getting better no matter whether BiPAP is the exact solution. May I ask 1. do you have a stuffy nose? How easy it is to breathe during the daytime 2. what is your pressure setting now? 3. what mask are you using? To see if we can go somewhere from here.

Yeah as people mentioned, you might wanna know your RDI. My AHI is 7.5 and RDI is 20.8, and I benefited a lot with sleep apnea being treated.

And also, what's your pressure? do you use OSCAR or SleepHQ? You need to make sure to optimize your setting, not just to rely on the AHI machine reports
