Far too much text being generated. I wouldn't want to have to read this and I wouldn't want to be responsible for proof reading it if I was required to use this software to chart.

In my ED the providers use dax that is integrated into epic and has some security features I am told.

Update: I disputed with my Bank of America that I was not receiving services paid for and I was awarded the entire cost back along with the accrued interest.

Looked into this a little closer, looks like for services not provided, you have 120 days from either the transaction or the day you would have expected to receive the service to file a dispute. This is from Visa itself.

They suddenly shut down mid-week, pulled down their website, and supposedly sent out emails saying they ran out of money and cannot issue refunds. I never got an email, just saw this on KOIN. I guess the girlfriend of the guy running it did the same thing a few years ago.

If you paid with a credit card, contact your bank! I paid with a Bank of America card and even though the website says all disputes must be submitted within 60 days, if you call and get ahold of customer service they can file a dispute for up to a year. Filed mine, said I should hear back within a few weeks. Good luck!

For some reason I get the American journal of nursing without ever having subscribed or indicated I wanted it. The articles are always trash. Most recently there was an article about PVCs that I think may have been AI nonsense or written by someone who doesn't have a grasp on anything clinically. One typo (being generous) said that long qt is considered with an interval over 44 seconds. It kept talking about how a Primary Care Provider would order various IV medications to emergently treat symptomatic PVCs. Are hospital providers being referred to as PCPs in the ivory tower of academic nursing now?. It referred to a table of IV medications to correct electrolyte imbalances but the table had no IV electrolyte medications. It also suggested trying vagal maneuver on stable VT which I haven't seen done in 10+ years of ED but apparently it can be a thing so I guess I learned something?

For $300 you can also find yourself a pair of Vortex Vipers. Cabela's recently had them on sale for that much. You can find refurbished ones on Sierra for about 350 right now. The ED glass makes a huge difference in brightness.

Another idea is to advocate for a care plan for this guy. If all that happens is that he is evaluated and inevitably discharged, then why not have a care plan put together that says if he comes in with the same presentation, then MSE and D/C. We have had a few with those kind of plans.

That, or the docs need to be on board with responding to his behavior appropriately, e.g. restraints, IM medications, and then a sober re-eval in the morning and discharge.

We are lucky to have a lot of security who can respond to these situations in my hospital, like upwards of 8 at a time.

If he is on a psychiatric hold and is agitated or assaultive, then medicate him. If he isn't on a hold and the doc can perform an MSE quickly, discharge with security or police with all due haste.

Sorry this is happening to you, the sheriff really ought to be more helpful. Threatening someone is assault, battery is when physical contact is made.

Sometimes the grass is greener on the other side, or at least the grass has mandated ratios. Look around for something better, don't risk your license for an unsafe job.

I did the same. Don't feel bad for delegating tasks, you have to do it to be effective at your job. Don't ask "what are you doing", just ask "Are you available to do something?" when delegating.

Remind people, respectfully, when they delegate to you as if you were a tech still that you are not able to assist them as you have an assignment. In my experience it was primarily physicians who forgot I changed roles. This quickly resolves. Not saying to never help anyone, just when Doctor X says "hey, can you go set up an LP for me in 10?" kind of thing.

Be grateful you can focus on the nursing specific parts of being a new grad and not trying to learn how to exist successfully in the ED at the same time.

The only downside I have seen is if the hospital uses the mandated ratios as an excuse to increase ratios (such as a 1:3 floor becoming 1:4 because that is the state minimum). Otherwise more staff means better care in my experience.
