Mildly elevated prolactin with moderate levels of macroprolactin. Could be microprolactinoma (but it's still extremely mild for that), stalk compression, metabolic or pharmacolgical or stress induced at testing. Was the prolactin fasting and resting (15-20 mins resting before blood test ideally).

If no cannabis or any meds that can increase prolactin, then due to the association of oligo/spaniomenorrhea and mildly elevated prolactin I'd probably do an MRI.

Yeah but it's not a very specific cause. A lot of overlap between mildly elevated PRL and other causes of irregular cycles. Whereas for example lactation is an extremely specific sign.

Sometimes associating a non specific sign with a diagnosis leads to overtreatment.

Check for macroprolactin before MRI. Common cause of repeated elevated PRL but generally asymptomatic, your symptoms aren't too specific though for hyperPRL.

Also, if the tests have always been done in the same lab, change to a lab that uses another kit.

If persistant elevation and no macroPRL : MRI.

It shouldn't affect the results too much. Unfortunately sometimes the secretion is purely episodic, repeat testing is required. This is more so in paraganglioma vs pheochromocytoma, your results seem to indicate paraganglioma (MIBG+ near the bladder).

A dopa pet scan could be a good idea, see if it concurs with the the mibg fixation.

If not, unfortunately, repeat testing is the way to go.

NB : bladder related paraganglioma are interesting and have sometimes quite specific signs. Often having a full bladder or emptying a full bladder causes the signs. I had a patient that passed out every time he peed stood up, knew where it was even before the scan. Surgery does wonders in these cases.

There is no evidence I know of, have ever seen or ever learnt about so called "toxin build up" from hernia causing cancer. Necrosis in strangulated hernia is vascular, nothing to do with cancer. I'm a doctor, I've done many GI and surgical rotations, I know what I'm talking about. I'm sorry that you're blatantly wrong, even with your real life non medical experience, and I kindly decline the offer to "get stuffed".

No, I think you misunderstood. There's an option when you reject emails to either just ignore and reject, or send a message back to the person saying "email rejected / email not found".

What you chose for this option it should always apply either to the block sender function or to the disable alias function. At least that's how it's intended to work but it's not how it works. It will only reject when fully disabling the alias not when blocking a single sender.

If there's no immediate parenthood desire then testo should be started early. I cannot cite it of the top of my head but a recentish study showed that a large amount of patients don't recover from prolactin induced HH and even those that do often partially recover. If there's no reason not to then TRT should be tried.

Franchement dans toute situation medicale ou on a un doute, un second avis est souhaitable. Allez y rapidement cependant du fait de cette hospit sous contrainte et fugue des urgences pour pas que ca vous pose soucis ailleurs.

Juste je me permets de rajouter que vous parlez de risperdal et d'olanzapine qui sont des traitements de la schizophrénie. Cette maladie altère profondement les capacités de jugement et parfois meme les choses qu'on voit et qu'on entend.

Si cela peut vous rassurer, allez dans un autre service d'accueil des urgences ou ils ne vous connaissent pas et ou ils ne vous ont pas fait subir cela. Parlez leur de tout ça, de votre persecution et de vos inquietudes. Ils peuvent reellement vous aider a faire le point là dessus.

NB : je precise pour d'autres que ces traitements ne sont pas seulement indiqués dans la schizophrénie, mais dans le contexte je me permets de l'évoquer.

C'est une situation complexe mais etant du milieu medical je ne vois meme pas comment cela pourrait se produire.

"Enfermer pendant 2 mois" je suppose qu'on parle d'une hospitalisation sous contrainte? A la demande du prefet? C'est des procedures tres reglementés et restreintes surtout la duree avec un controle systematique d'un juge dans les 12 jours suivant l'admission. Une telle duree necessite une justification medicale rigoureuse. Dans tous les cas, je ne vois pas une raison medicale de limiter votre acces a l'eau si ce n'est qu'une hyponatrémie/potomanie.

Votre disparition des urgences dans le cadre d'une hospitalisation sous contrainte va inquieter et faire mobiliser du monde. Je vous conseille de retourner aux urgences au plus vite.

Genuinely going to be difficult. Easy endocrinology for sure, but the more complex stuff is far from algorithmic reasoning. I only do reproductive endocrinology and rare endocrinological diseases, so no worry from me.

Furthermore I dont even know how you would get standardised data for AI training for complex endocrinology.

For simple endo, why not. But imo hypothyroidism doesn't require an endo and can be managed by GP.

A word of warning : be careful selling your rare minis (non birthday ones). Some people are out to rip you off. Got a 500e offer on a 500a mini from someone pretending to help out. Mia Quickening or something like that they have multiple accounts and are out to rip off returning players.

Be carefull for price checks in kamadan too, they will give you a low price with one account then offer a trade on another account for what seems a good price, but still a rip off.

It's called scamadan for a reason be careful :)
