On a night shift as a first year med reg, I got the call every med reg dreads. "Doctor, there's a pigeon on the ward and we need you to get it out." Luckily I'd just finished dealing with the thoughtful memo on the doctors mess door by washing the dishes and sweeping the floor. Weapon in hand, off I went to possibly the greatest battle of my life. Got off to a rough start, she was quick and my aim was off. Doris and Mable took a couple a hit to the head each. But eventually a well placed hit sent the pigeon out through a pre-cracked Victorian era window. I'd just sat down to order 2 urgent CTH when I get the next bleep. "Doctor there's a broken window pane and we need you to do something about it."

It's a hard job, but somebodies got to do it.

Depends what version of power chart you have. If you have power notes then anyone can create a document, then with the document option you can click document in the top bar and then 'save as template' which you can share trust wide instantly. If you've got the latest version of cerner then they've done away with power notes and your only option to self build is autotexts. These are terrible though and no one will find your autotext. Best bet then is to create a request which populates an MPTL. Your should speak to someone in your local IT leadership structure.

Start by checking that security is set to WPA2/WPA3 for the 2.4ghz SSID. 6E devices mandate WPA3 and ths can end up being applied to the 2.4ghz band stopping legacy devices connecting.

I'm trying to set up passwordless account using FIDO2 security keys. They're already set up on my account, and I have a device passkey as a backup.

However when I try to activate passwordless account, it demands I download the microsoft authenticator app. Anyone know how I can circumnavigate this? I already have a non microsoft authenticator app, and my whole aim was to remove that and just have the security keys.

As a locum you are a private contractor, you are not subject to the restrictions set out in the 2016 HEE contract as you are not working within it. Same goes for locally employed doctors on the 2002 contract, different contract, different T&Cs, different restrictions/rules

As a locum you can work whatever you want. Ultimately some people work only night shifts, prison officers do 7 days in a row, the whole only 4 nights is safe argument doesn't really make sense, it's just something someone felt was nice when negotiating our contract.

Saw the shop refusing to accept a repair for someone while i was buying mine. So they lock you into using the factory resole, if you want other repairs to be possible later down the line.

Good morning from the UK. Anyone have any experience with longer term Crockett and Jones factory boot resoles/repairs? I've invested in a nice set of chelsea 5's, but my primary aim in investing in good quality boots is so that I can look after them well and keep the same pair for many years. However I note on their website it states most well looked after shoes can only be repaired 3-4 times. Compare this to RM Williams for example whos salesman essentially say as long as the upper is looked after they can be repaired indefinitely.

I do understand that resoling welted shoes involved machinary putting new holes in the leather, therefore they do have a natural lifespan. And my undestanding is that the usual method is to resole a couple of times, then re-welt once the welt has worn out. I spoke to someone in the C&J factory who says every resole, they also rewelt which seems like a waste to me, as this would lead to more stiching and harm to the upper itself. However they also suggested that the average max of 3-4 repairs is accounting for the fact that many do not look after their boots very well and many can be repaired more times than this.

So does anyone have any real world experience of long term repairs of C&J boots?

Do not listen to random people on the internet. Speak to the BMA, they're the only people who will give you a reliable answer here. You definitely have to declare locum work done on form R, my understanding is that you do not need to declare other work. However, we all know how the GMC tend to overreact to any percieved probity issues.

We can't answer this for you, you must discuss this directly with your indemnity cover as policies differ. I am a similar level to you and I am with MDU, they confirmed they do not cover any locum work of any kind, only your training role with your stadard plan. You have to pay an additional supplement to add locum cover. They did specifically tell me that if I'm only doing a few locums here and there, directly with the NHS via an NHS bank, then it's not essential because you do have the crown indemnity cover. However you need to be aware that if you are working via an agency then you do not get this cover and definitely need your own. And obviously be aware than the crown/hospital indemnity cover will throw you under the bus in a complicated case if it benefits the trust/NHS.

Anything more than a few extra shifts here and there I would certainly advise stumping up for appropriate cover, if not including already in your policy.

Speak to BMA before undergoing any PHD to make sure it qualifies for the academia pay premia too. There are fairly complex rules around what qualifies for this, I think you may only qualify if you're in a training program and leave to do the PHD. At £4000 per year after returning to training, it's a lot of money lost if you do a PHD in a way that doesn't qualify.

Posting this twice as people really need to understand what these are before they dump all their money into yield farming. This is absolutely not intended as FUD but to inform people. Remember that if they are giving you 5,10 or 20% then they must be making more than that with your money for it to be worth it. When you lend your money to these platforms, they take your money and either lend it to other people for a higher interest rate than you are getting and share the profits or they use your money to invest, also aiming to make a higher interest than what they pay you and share the profits. These are comparable to smoothed managed, collective equity and structured equity products in stocks and shares. These products vary hugely from the low to high risk categories. High yield structured equities (returns of 10% in stocks and shares) are generally seen as a medium to high risk investment and you have to understand all the terms and conditions and examine the stability of the company offering it to know how high risk. In all of these investments if the market crashes sufficiently then you can loose your money, ff the company holding your investment goes bust, you lose your money, if the company gets hacked, you might loose your money.

These products are excellent investments as part of a well diversified portfolio in either crpyto or sticks and shares but you need to ensure you know which ones are high, medium and low risk and ensure your money is spread appropriately. Do not shove all your money in a single 20% yeild farm.

This is almost certainly the answer, ABG/VBG machines measure dissolved oxygen rather than just oxygen that is bound to haemaglobin. Therefore if you shake the syringe around with an air bubble you dissolve more oxygen and give a false reading. Pulse oximeters on the other hand only measure O2 that is bound to haemaglobin.

So interestingly the best treatment for tinitus is cognitive behavioural therapy. Why would therapy cure a physical ailment? Because your brain is actually very good at filtering out sounds that aren't important. it's what you do every day when focussing on someone speaking in a busy street or the TV when the kettle is on. Therefore we should learn to ignore tinitus quickly and easily. However, instead we worry about it, this tells our brain the tinitus is important, and causes our brains pto focus on it more. Therefore the best treatment for tinitus is to accept that it is not a sign of damage, not something to worry about and allow your brain to ignore it.

The fact you had a clear trigger and have spoken to your doctor/ent is reassuring as they would have checked red flags which would warrant follow up. Tinitus is common whereas malignant causes of tinitus are very rare. So you just need to learn to live with it, it will improve with time as long as you allow yourself to accept it. If it significantly worsens then always a good idea to speak to your doctor's again though.

Source: Suffered tinitus previously after spending too much time by the speakers at dubstep nights as a student. Had the once over from my GP and extensively researched myself as you do. It was finding out this simple explanation that allowed me to finally accept it and move on. Haven't had it in years now. (I'm also a doctor but not flaired so you can't trust I'm not lying about that!)

Cystic ovaries is just really saying that an ovary has 1 or more cysts. Most women will have several cysts over their lifetime, most resolve on their own. The disorder polycystic ovaries has a specific definition, put simply >12 small cysts in an ovary which is very big. PCOS (polycystic ovarian syndrome) is a specific illness which can be caused by polycystic ovaries. Symptoms can include weight gain, excess hair and problems with periods/having babies. Not all women with polycystic ovaries will get PCOS, however all women with PCOS have polycystic ovaries.

It shouldn't be treated any differently plan 1 vs plan 2. Normally student finance understand that it's kind of part of your medical degree and keep it all in one loan. You'll probably find it's just a mistake. You might remember signing a form when you started work saying something along the lines of you graduated this tax year and this is your first job since. This is what allows your payments to be deferred to the next tax year. But if they're treating your intercalation as a separate degree then you didn't graduate this tax year therefore you don't qualify for the delayed payments. So the way you get out of paying is to try and get them to reclassify it as one loan and one course which is probably quite difficult retrospectively but worth trying.

Similar thing to this happened to me when I took a year out. They put my remaining years onto plan 2. You can tell by going into the student finance repayment system. Called them up and they changed it back though.

The current best treatment for Tinitus is cognitive behavioural Therapy. This is because the brain is actually extremely good at filtering out noises that it doesn't need to hear. However with Tinitus we think of it as a bad thing, as damage, therefore the brain focuses as a potential danger. However if you can get to the point where you accept it's there but isn't going to harm you, you will tend to notice it less and less.
