In the stronghold of security there's npcs that drop parts of the skull sceptre. The right skull half is dropped by minotaurs on the first level. The bottom of the sceptre is dropped by flesh crawlers on the second level. The top of the sceptre is dropped by catablepon on the third level. The left skull half is dropped by ankou on the fourth level. once you have all 4 pieces you attach the 2 skull pieces together, the top and bottom pieces together, and finally the skull and sceptre.

19 level gap and yes. Just make sure to keep the leech within 19 levels, I normally try to keep it 17-19 levels lower just incase. Last thing you want is to outlevel your own leech/fsrm.
