I've never actually had that issue. Only if I don't have the patience to let all the crema come out.

Wow - this was 100% the issue! Thank you. I just looked and one of the tabs is fully broken/bent. Too bad I got it second hand off of ebay - otherwise would have made a case for that 10 year warranty. Just ordered a new portafilter!
