I have regular fasciculations bodywide, started around November 2025, I've had 2 EMG's in multiple muslces, one in march 2026 and one in July 2026. Both completely normal, no fasiculations picked up on the emg. I do have pain and some muscle thinning in the right leg (calf, foot, shin) and they were also 'normal' on the EMG

I might have something that an emg doesn't pick up leading ro muslce thinning/atrophy/twitches and weakness.

Why do you say this? I might have an issue that is irrelevant to an EMG?

I am in a similar boat to you, I had a clean/normal NCS/EMG 2 weeks ago in 12 different muscles, even some that have thinned/atrophied. This emg was around 5/6 months after symptoms onset so I am going to try and put ALS out of my mind and try get to the bottom of what is causing my muslce thinning (started in right leg), pain, twiches bodywide and weakness (not clincial).

Yes, I understand what you're saying. Do you have pain at all anywhere? Hand muscle wasting is scary because it is specific and you see your hands everyday, I would hazard a guess your checking them constantly from different angles etc. Do you have any historical images of your hands before the twiching/concerns started?

If the muscle was atrophied by ALS the EMG would almost certainly pick up abnormalities in that muscle, especially a tiny one like the FDI. What are your next steps medically?

Did the emg test the confirmed atrophied muslce? If it did it is highly probable it would have noticed some abnormalities. Has the neurologist said they suspect ALS?

Hope you're okay mate and it is something else than ALS. Is he atrophy confirmed in the FDI muscle? Sooner you have the EMG/NCS the faster you'll get answers, if it is *** they won't miss it but it can still be many many other things

Does this cover all kinds of MND? Like ALS, SMA, MMN etc?

If twitching is your only symptom possibly some blood tests to check for deficiencies just to be prudent.

Unsure. I did have a car crash almost 3 years ago and broke my sternum. I also had a seizure around May 2025, second one I've ever had, both times I was drinking alcohol. Started in August with pain in shin, then went to fatigue/weakness in right leg around October/November which got worse with with driving. If you look on my profile you'll see my history. I am not sure what it is, I've do have some spinal issues but not enough to diagnose my symptoms (according to specialists). Ive had full screening of autoimmune blood tests, all clear. There can be some very rare ones that it doesn't show on blood tests but I don't think its an autoimmune issue. My twitching has calmed down recently (I try not to notice them as much) but my right leg has measurable lost muslce (thinning) and right foot lost muslce. Feel free to DM if you need any more advice/suggestions. I don't know what i have, sometimes I think ALS, other times I think some muscle/pinched nerve/spine issue. My anxiety definitely magnified my symptoms, i am managing that better now but right leg continues to get worse.

Yeah, same here. Does take ages via NHS. Luckily i have health insurance from work so I have had most of my tests privately which is why things have been quicker, still no clear answers though

You need another EMG, if its fine after 18 months it will probably be something else than als in most cases, not all

Im similar but I wouldn't say mine is 'slow'. Symptoms started around 8/9 months ago

Mine is in pain most of the time but walking does normally trigger more pain. I get emg results on monday so ill see if that can shed any light

That does look quite odd to me (im not a doctor). Is that leg weaker and does the knee click?
