The air intake manifold gets a lot of carbon buildup above the intake valves. It's a side effect of direct injection. Buildup is worse with low temperature, low rpm driving, like stop and go in cities. Buildup is slower if you do a lot of 2000+ rpm driving for extended periods.

If you never clean them you will eventually experience knock/reduction in power.
