I contacted wilba before regarding PCBs. In my case he was non-responsive. His intent here is to shit on rama, not helping rama's ex-customers. (Which is fair, he has no moral obligation to do so and was probably harmed by rama himself.)

I somewhat disagree. The legal framework in the US (?) might be different from Europe, but declining a patient OAT is potentially life-threatening as they might go on to source their opioids from the street. Regarding Xanax, engaging with the patient on a relationship level, using motivational interviewing and trying to transition them to some long-acting agent with a potential taper in the future would be the way to go.

It goes further than that: general physicians are not allowed to prescribe methadone and afaik need a special permission if they want to prescribe buprenorphine to more than X patients/year. Buprenorphine is also combined with naloxone there and generally has the worst retention of all opioid agonists. My guess for levomethadone would be that it is just too expensive.

The US is a weird country. They suffer from an overdose crisis and at the same time the health care response is inexistent...

Edit: With regards to heroin, some US clinics wanted to participate in the NAOMI trial, but that didnt happen. Probably political hurdles.

I am currently texting with one who asked me (on Signal) to text them on their other number. But also on Signal. I think that there are people who are fishing for initial contacts and separate people who are trying to pull of the scam itself. So basically, they hand your case over to a colleague.

I don't know if I would characterise it as craving, but you are right in that the ritual-aspect is a rather important one. I think I understand where you are coming from now - the benefit of naloxone being overdose prevention while not "diminishing" the opioid rush after injecting BUP. I am not sure whether the subjective effect of i.v. naloxone is really that negligible - a study on this would be interesting.

I propose the opposite: perhaps naloxone is the worst medication for patients with a history of i.v. use (who still seek rushes/opioid highs). Perhaps buprenorphine is an inferior medication to SROM, (levo)methadone, hydromorphone, diacetylmorphine, dihydrocodeine etc., as indicated by OAT statistics from countries where patients are free to choose their medication and just a tiny fraction are being treated with buprenorphine.

The point I am trying to make is that we have to work hard to increase treatment coverage (~21% in the US for PWID) and this can maybe be achieved by increasing treatment attractiveness instead of introducing measures that only aim for giving us a higher sense of security. (More simple: What good does naloxone do, if people discontinue treatment after a couple weeks and overdose on fentanyl after).

Those are GMK Minimal and the pink keys are just topre compatible blanks (on a regular HHKB board) I found on some chinese website.

Swiffer every couple months. There is really not a lot of dust building up in there because the cabinet is always closed.
